Recently, the Chief Justice of the Supreme People's Court of Vietnam has issued the Circular No. 04/2020/TT-TANDTC guiding the process of appointment, re-appointment, dismissal, commendation and handling of violations; issuance and withdrawal of mediator’s card.

Issued together with the Circular No. 04/2020/TT-TANDTC of the Chief Justice of the Supreme People's Court of Vietnam are 18 documents and forms on appointment, dismissal, commendation and handling of violations; issuing and withdrawing mediator’s cards, specifically including:

- A request for appointment of the mediator (Form No. 01);

- A request for re-appointment of the mediator (Form No. 02);

- Decision on appointment and re-appointment of mediator (Form 03);

- A request for dismissal of the mediator (Form 04);

- Decision on dismissal of the mediator (Form No. 05);

- Proposal for termination of work as a mediator (Form No. 06);

- Decision on termination of work as a mediator (Form No. 07);

- Mediator’s card (Form No. 08);

- Application form for issuance of mediator’s card (Form No. 09);

- Application form for renewal or re-issuance of mediator’s card (Form No. 10);

- Application form for appointment (Form No. 11);

- Application form for re-appointment (Form No. 12);

- Brief curriculum vitae (Form No. 13);

- Minutes of the Advisory Council meeting (Form No. 14);

- Resolution on selection, appointment/re-appointment/dismissal or handling of violations of the mediator (Form No. 15);

- List of excerpts of request for appointment/re-appointment/dismissal of the mediator (Form No. 16a, Form No. 16b, Form No. 16c);

- Report on the performance of tasks during the term of the mediator (Form No. 17);

- Assessment of comments of the Court where the mediator works on the process of performing the task (Form No. 18).
