On July 11, 2017, the Ministry of Education and Training signed Decision 2329/QD-BGDDT promulgating the Survey Toolset and Guiding Document for Implementing the Measurement of Public Satisfaction with the Quality of Public Education Services.

The toolset comprises a survey questionnaire with the following 06 main contents:

- Access to services;- Facilities and equipment;- Educational environment;- Educational activities;- Educational outcomes;- Other opinions.

It includes 8 survey forms:

- Form P1: For parents of preschool students;- Form P2: For parents of primary school students;- Form P3: For parents of lower secondary school students;- Form P4.1: For parents of upper secondary school students;- Form P4.2: For upper secondary school students;- Form P5: For students of pedagogical colleges;- Form P6: For learners at continuing education centers;- Form P7: For students of colleges of pedagogy and universities.
