For birth months from March to September, use a single digit, and for other months, use two digits;- Field “Gender”: For male, write “Male”; for female, write “Female”;- Field “Ethnicity”: Record the ethnicity of the citizen as per the BC;- Field “Nationality”: Record the nationality of the citizen as per the BC or the document certifying Vietnamese nationality issued by the competent authority;- Field “Registered place of birth,” and field “Hometown”: Record in accordance with Points h and i, Clause 2, Article 7 of Circular 66/2015/TT-BCA;- Field “Permanent residence”: Accurately and fully record according to the household registration book.
