Forms for statement and reports on occupational accidents in the Military in Vietnam

On January 04, 2017, the Ministry of National Defense issued Circular 01/2017/TT-BQP regulating the statement, investigation, statistics and reports on occupational accidents within the Ministry of National Defense of Vietnam.

Issued with this Circular are the 12 following appendices:

- Appendix I: List of injuries to determine the type of severe occupational accidents;

- Appendix II: Occupational accident declaration form;

- Appendix III: Decision template for the establishment of an occupational accident investigation team;

- Appendix IV: Decision template for the establishment of an occupational accident investigation team under the Ministry of National Defense of Vietnam;

- Appendix V: Testimony record template;

- Appendix VI: Occupational accident investigation record template (minor or severe);

- Appendix VII: Occupational accident investigation record template (severe or fatal);

- Appendix VIII: Minutes of the meeting to announce the occupational accident investigation record;

- Appendix IX: Handover and receipt record template for documents, items, and means related to the occupational accident;

- Appendix X: Occupational accident statistics form;

- Appendix XI + XII: Summary report template for occupational accidents at unit level and under the Ministry of National Defense.

Circular 01/2017/TT-BQP takes effect from February 20, 2017, and replaces Circular 147/2012/TT-BQP.
