On August 19, 2024, Vietnam Social Security issued Decision 1449/QD-BHXH regarding the Procedure for receiving and resolving files for two groups of electronic interconnected administrative procedures: Birth registration, permanent residence registration, issuance of health insurance cards for children under 6 years old; declaration of death, deregistration of permanent residence, settlement of funeral expenses, and survivor benefits in Vietnam
- Interconnected application for the administrative procedures of declaration of death, deregistration of permanent residence, settlement of funeral expenses:
+ Electronic declaration form No. 02;
+ Electronic copy of Death Extract.
- Interconnected files for the administrative procedures of death registration, permanent residence deletion, and one-time funeral expense and survivor benefit settlement:
+ Electronic declaration form No. 02;
+ Electronic copy of Death Extract;
+ Confirmation information from relatives agreeing to appoint a representative to receive the one-time survivor benefit via the VNeID application (displayed in Column 11 of the electronic declaration form regarding the list of the deceased's relatives as stipulated in the Social Insurance Law).
In cases where relatives cannot confirm through the VNeID application, it should be replaced with an electronic copy of the Declaration Form of Relatives (Form No. 09-HSB).
- Interconnected files for the administrative procedures of death registration, permanent residence deletion, and monthly funeral expense and survivor benefit settlement:
+ Electronic declaration form No. 02;
+ Electronic copy of Death Extract;
+ In cases where the relative is a child aged 18 or older, a wife under 55 years old, a husband under 60 years old, or the biological parents, the biological father of the wife or the biological father of the husband, the biological mother of the wife or the biological mother of the husband, other family members whom the social insurance participant is obliged to support according to the marriage and family law, under 60 years old for men, under 55 years old for women, without a monthly income, or have a monthly income lower than the statutory pay rate and have a reduction in labor capacity from 81% or more, the following documents should be attached:
++ An electronic copy of the Medical Assessment Report on the reduction in labor capacity from the Medical Assessment Council for those reduced in labor capacity by 81% or more, or a previous Medical Assessment Report that qualifies for other policies, or a Disability Certificate equivalent to a reduction in labor capacity of 81% or more.
++ Invoices, fee receipts for assessment, and a list of assessment contents from the medical assessment facility in cases of reimbursement for medical assessment fees.
