Currently, according to Circular 07/2014/TT-BVHTTDL, the assessment conclusion form is one of the samples of documents to carry out judicial expertise in the field of culture in Vietnam.

The form of assessment conclusion includes form No. 04a and Form No. 04b.

Now, Circular 01/2022/TT-BVHTTDL has issued a new assessment conclusion form (Form 04b) to replace form 04b issued together with Circular 07/2014/TT-BVHTTDL.

At the same time, add a sample of the minutes of examining the object of expertise at the place of storage of the person soliciting or requesting an assessment (Form 06).

In addition to the samples mentioned above, the sample document for carrying out judicial expertise in the field of culture in Vietnam also includes:

- Form of minutes of handing over and receiving dossiers, subjects of solicitation and request for expertise (Form No. 01);

- Form of minutes of unsealing opening (Form No. 02);

- Sample document recording the assessment process (Form 03);

- Form of minutes of handing over assessment conclusions (Form 05).
