The Ministry of Health has just issued Circular 56/2017/TT-BYT detailing the implementation of the Law on Social Insurance and the Law on Occupational Safety and Hygiene in the field of health.

The templates applied for the procedures to enjoy maternity policies and assessment examination procedures to enjoy social insurance (BHXH) policies attached to Circular 56 include:

- Appendix 1: Sample referral recommendation for assessment;- Appendix 2: Sample examination request form for assessment;- Appendix 3: Sample hospital discharge form;- Appendix 4: Sample summary of medical records;- Appendix 5: Sample birth certificate form;- Appendix 6: Sample maternity leave certificate form;- Appendix 7: Sample leave certificate form to enjoy BHXH;- Appendix 8: Sample registration form for seal and signature to be used to certify the leave certificate to enjoy BHXH.

Note, the leave certificate form to enjoy BHXH, hospital discharge form, and birth certificate form issued before the issuance date of Circular 56 will continue to be applied until December 31, 2018.

Circular 56/2017/TT-BYT takes effect from March 1, 2018.
