16 forms of documents used to register adoptions involving foreign elements in Vietnam

Circular 07/2023/TT-BTP regulates 16 forms of documents used for registration of adoptions involving foreign elements as follows:

- Application to adopt Vietnamese children abroad (Used for adoption of children living in foster care facilities, bilingual Vietnamese - English)

- Application to adopt Vietnamese children abroad (Used for adoption of stepchildren, biological grandchildren, bilingual Vietnamese - English)

- Documents on notable characteristics, interests, and habits of adopted children

- Document certifying that the child is eligible to be adopted abroad

- Decision on adoption involving foreign elements (Original copy - with content)

- Decision on adoption involving foreign elements (Original copy - embryo, no content)

- Extract of adoptions involving foreign elements (Copy)

- Certificate of adoption involving foreign elements in accordance with Hague Convention No. 33 (Bilingual Vietnamese - English) (Original copy - with content)

- Certificate of adoption involving foreign elements in accordance with Hague Convention No. 33 (Bilingual Vietnamese - English) (Original copy - blank, no content)

- Report on the development of Vietnamese children adopted abroad (Bilingual Vietnamese - English)

- Application for grant/extension/amendment of operating license of foreign adoption organization in Vietnam (Bilingual Vietnamese - English)

- Operation license of foreign adoption organization in Vietnam (Original copy - with content)

- Operation license of foreign adoption organization in Vietnam (Original copy - blank, no content)

- Report on the operations of foreign adoption organizations licensed to operate in Vietnam (Bilingual Vietnamese - English)

- Extract recorded in the Book of adoptions that have been resolved at a competent foreign agency

- Extract recorded in the Book of adoptions that have been resolved at a competent foreign agency (Copy)
