15 forms relating to the management of specialized certification services in Vietnam from February 18, 2020

Recently, the Ministry of National Defense of Vietnam issued Circular No. 185/2019/TT-BQP providing guidelines for provision, management and use of specialized certification services for governmental authorities.

DOWNLOAD 15 FORMS UNDER ANNEX ATTACHED TO CIRCULAR NO. 185/2019/TT-BQP

Promulgated together with Circular No. 185/2019/TT-BQP of the Ministry of National Defense of Vietnam is the Annex including 15 forms relating to provision, management and use of specialized certification services for governmental authorities that shall be used from February 18, 2020. In particular:

- Form 01: Written request for issuance of an individual's digital certificate;

- Form 02: Written request for issuance of an individual's digital certificate made by supervisory authorities;

- Form 03: Written request for issuance of a digital certificate to an agency or organization of the person assigned to manage the digital certificate by the agency or organization;

- Form 04: Written request for issuance of digital certificate to agency, organization made by supervisory authorities;

- …

- Form 14: Record of transfer and receipt of security device of expired or revoked digital certificate;

- Form 15: Record of confirmation of loss of security device.
