13 cases denied processing reflect COVID-19 vaccination information in Vietnam

According to the Official Dispatch, there are 13 cases of submission of feedback that are not reviewed/rejected to reflect vaccination information, including:

(1) Photograph of the screening form but no information on the date of injection, the injection and the type of vaccine;

(2) The information reflected with the injection certificate is opposite of the First and Last Name information;

(3) Only attach a personal photo instead of a vaccination certificate;

(4) Wrong information on injection declared on Portal and information on injection certificate;

(5) The date of birth on the certificate does not match the date of birth on the system;

(6) The certificate of shooting does not contain information such as full name, gender, time of injection, vaccine;

(7) A photo of the certificate but hides personal information;

(8) The photo is blurred and the text cannot be read clearly;

(9) The injection date information on the Portal is different from the information on the injection certificate;

(10) All information on the certificate and declared information do not match;

(11) Full name on the Input Gate is incomplete, written without accents;

(12) Declare injectable vaccines with different certification information;

(13) Personal information is edited on the ticket.

(People can look up result information on the Portal and receive a notice asking people to return to the injection facility to reflect and be updated with accurate information).
