The Ministry of Health has just issued Decision 5772/QD-BYT promulgating the form and process for the issuance of "Vaccine Passport".
Accordingly, the form and process for issuing the "Vaccine Passport" issued under Decision 5772/QD-BYT are applied uniformly at all vaccination facilities across the country.
"Vaccine passport" form
- Information fields to display:
1. Full name;
2. Date of birth;
3. Nationality;
4. Diseases to which the certification targets;
5. Number of injections received;
6. Date of injection;
7. Dose No.;
8. Vaccines;
9. Vaccine products;
10. Supplier or manufacturer of vaccines;
11. Code of the certificate.

The above information will be digitally signed, encrypted and packaged as a 2D QR code. Specific requirements will be described in Section 4, Forms and procedures for issuance of “Vaccine passports” issued together with Decision 5772/QD-BYT .
- The above information includes full name and date of birth combined with other identification documents such as ID card, CCCD or passport to help identify the owner.
- Information about the disease for which the certificate is targeted, vaccine, vaccine type and supplier or manufacturer will be displayed in correspondence with the updated WHO “COVID-19 vaccine tracker and landscape” document on the website. WHO web portal and “Value Sets for EU Digital COVID Certificates” issued by the European Union (EU)
- Date of injection and number of injections received to identify vaccination information.
- QR Code expires after 12 months from the date of creation.

Step 1 : Immunization establishments review, verify and authenticate information about people getting vaccinated against COVID-19 according to the instructions in Official Letter 8938/BYT-DP dated October 21, 2021 of the Ministry of Health on the instructions on the process of information verification and vaccination against COVID-19 and Official Letter 9438/BYT-CNTT dated November 5, 2021 of the Ministry of Health on guiding the process of authenticating people's information on the Management Platform. vaccination against COVID-19.
Step 2 : Immunization facilities digitally sign COVID-19 vaccination data on the COVID-19 Immunization Management Platform. The COVID-19 Immunization Management Platform connects and shares vaccination data with the COVID-19 Vaccine Immunization Certification Management System that meets the regulations on connecting medical data set by the Ministry of Health promulgate. The COVID-19 vaccine vaccination data on the COVID-19 Immunization Management Platform needs to meet the requirements in Sections 3 and 4 of the "Vaccine Passport" form and process issued together with Decision 5772/ QD-BYT .
Step 3 : The Department of Preventive Medicine - Ministry of Health shall digitally sign the certificate of vaccination against COVID-19 concentrated. The certificate is issued using the EU standard QR code format specified in Section 5 of the "Vaccine passport" form and process issued together with Decision 5772/QD-BYT .
National epidemic prevention applications and other utility applications (if approved by the individual user) receive and store the confirmation of vaccination against COVID-19 vaccine in the form of a QR code according to the instructions for delivery. change medical data issued by the Ministry of Health.
Decision 5772/QD-BYT takes effect from December 20, 2021.
