Form of Regulation on grassroots democracy at work (Form 04) is issued together with Guide 41/HD-TLD on November 11, 2021 on trade unions participating in dialogue and implementing grassroots democracy regulations in the workplace.

The Form of Regulation on democracy at grassroots in the workplace consists of 4 chapters and 17 articles with 03 main contents:
- Contents of the employer's publicity, employees' participation in comments, decisions, inspection and supervision; (Articles 5 to 10)
- Organize employee conference; (Article 11)
- Organize dialogue at the workplace (Articles 12 to 16)
In addition, promulgated together with Official Guideline 41/HD-TLD of Vietnam dated 11/11/2021, there are some sample documents as follows:
- Form 01: Minutes of periodical dialogue / upon request / when there is an incident;
- Form 02: Resolution of the year employee conference...;
- Form 03: Minutes of the employee's conference in...;
- Form 05: Summary of data on results of the implementation of the Regulation on democracy at grassroots level for 6 months……/……
Based on the characteristics, labor situation, production and business activities, and size of the enterprise, the trade union shall proactively propose to the employer to develop or amend and supplement the Regulation on democracy at the grassroots level. at the workplace (hereinafter referred to as the Regulations).
Employers employing less than 10 employees are not required to issue Regulations.
Official Guideline 41/HD-TLD of Vietnam was promulgated on November 11, 2021 and replaces Official Guideline 1360/HD-TLĐ dated August 28, 2019.
