On August 1, 2017, the Ministry of Health of Vietnam signed Decision 3480/QD-BYT on the Plan for HIV load testing for the period 2017-2020 with the following specific objectives and activities.

Goal 1: 95% of HIV/AIDS patients at treatment care facilities nationwide will be routinely tested for HIV load by 2020 in Vietnam |
Targets |
- Increase the percentage of HIV/AIDS patients under treatment at HIV/AIDS care and treatment facilities who have access to routine HIV load testing from 70% in 2017 to 100% by 2020.
- 50% of HIV/AIDS patients under treatment at HIV/AIDS care and treatment facilities with health insurance coverage will have routine HIV load tests paid for by health insurance by 2018.
- Conduct viral load testing using mobile equipment or dried blood spot techniques for deep and remote areas with limited access to medical services.

Activities |
Increase access rates for routine HIV load testing
- Update guidelines for HIV load testing in HIV/AIDS treatment monitoring.
- Provide counseling and education for healthcare workers and patients about the benefits of HIV load testing in ARV drug treatment monitoring.
- Enhance the capacity of healthcare staff at various levels concerning HIV load testing.
- Conduct communications to change behavior and raise awareness among HIV/AIDS patients about the importance of using HIV load testing.
- Appraise and evaluate the equivalence of HIV load tests using mobile devices at Institutes/Hospitals and reputable organizations.
- Implement pilot programs and expand the use of mobile equipment (POC VRL) suitable for geographical conditions.
- Make the most of the existing testing equipment systems capable of performing multiple diagnostic tests for different infectious diseases.
Increase health insurance coverage for routine HIV load testing
- Coordinate with health insurance agencies to develop and issue guidelines on payment for transporting HIV load test samples.
- Develop and implement health insurance payment for HIV load tests.

Goal 2: By 2020, 20 facilities will conduct HIV load testing, and 100% of these facilities will meet national or ISO 15189 quality standards in Vietnam |
Targets |
- Establish one system of central testing facilities in major cities and specialized medical centers.
- 100% of HIV/AIDS care and treatment facilities will be trained in sample collection, preservation, packaging, and transportation for HIV load testing.
- 100% of HIV load testing facilities will maintain test quality according to national standards.
- At least 50% of HIV load testing facilities will be certified to meet ISO 15189 standards.

Activities |
- Add 13 more testing facilities to the network of HIV load testing centers for Central, Tay Nguyen, North, and Mekong River Delta regions by 2020.
- Establish a coordination mechanism between state and private laboratories, hospitals, research institutes, and the patient referral or sample transportation system.
- Provide training to ensure the quality of sample collection, packaging, preservation, and contracting specialized sample transportation to facilities and performing tests at central laboratories according to regulations.
- Strengthen and develop the national reference laboratory for HIV load testing.
- Research and produce external quality control panel samples, proactively supplying external quality control samples to HIV load testing facilities.
- Develop national guidelines on managing and ensuring the quality of HIV load testing, and establish national standards for HIV load testing.
- Develop standard toolkits for inspecting and evaluating laboratories to improve quality towards achieving national and international quality standards.
- Manage and supervise to ensure test quality at HIV load testing facilities according to Ministry of Health regulations.

Goal 3: 90% of HIV load testing facilities will have a two-way information exchange system with HIV/AIDS care and treatment facilities and specimen-sending facilities in Vietnam |
Targets |
- 90% of HIV load testing facilities will exchange information two-way with HIV/AIDS care and treatment facilities sending specimens.
- The time for returning HIV load test results to HIV/AIDS patients will not exceed 14 days.

Activities |
- Develop regulations on managing HIV load test information.
- Establish and apply laboratory information management systems at HIV load testing facilities.
- Add information on the activities of HIV load testing in periodic HIV/AIDS prevention and control activity reports and the national monitoring and evaluation index set.
- Connect laboratory information systems with HIV/AIDS patient management information systems.

Goal 4: 100% of facilities conducting HIV load tests and care and treatment facilities will implement an HIV load testing reporting system in Vietnam |
Targets |
- Establish one technical team to monitor and evaluate the activities of HIV load testing laboratories.
- Develop and implement an HIV load testing reporting system nationwide.

Activities |
- Update indicators and the reporting system to monitor and evaluate HIV load testing activities in the monitoring and evaluation index set of the HIV/AIDS prevention and control program.
- Collect and analyze national data to monitor and evaluate the program, provide support supervision, and improve the quality of the care and treatment program.
- Develop plans and organize the monitoring and evaluation of the HIV load testing process, including sample collection, transportation, and ensuring test quality.
