In order to assist our Esteemed Customers and Members in conveniently registering for the high school graduation exam and applying for University and College admission, Thu Ky Law hereby sends to our Esteemed Customers and Members detailed instructions on how to fill out the registration form for the high school graduation exam and applying for University and College admission as follows:

Section DEPARTMENT OF EDUCATION AND TRAINING CODE: The candidate registers at the unit under any department, write the name of that department in the blank space, then fill in the 2-digit code in the next two blank boxes. The department code is regulated by the Ministry of Education and Training.
Section Registration Number: To be filled out by the registration receiving unit; candidates do not fill in this section.
Section 1, 2: Follow instructions on the High School Graduation Exam and University Admission Registration Form (abbreviated as Registration Form).
Section 3:
a) Candidates only need to specify the province or city of birth. For those born abroad, only the country name (in Vietnamese) is required.
b) Ethnicity as per the birth certificate.
c) For foreign nationality, mark (X) in the adjacent box.
Section 4: For the old ID model, fill in the last 9 digits, leaving the first three boxes blank. For the new ID model or Citizen Identification Card, fill in all 12 digits in the corresponding boxes.
Section 5: Province (city) code, district (town) code, and commune (ward) code, as regulated by the Ministry of Education and Training. Candidates need to check at the registration unit to fill in the correct codes for the province (city), district (town), and commune (ward) where their current household registration is located. If the candidate does not have a household registration at an Area 1 commune, leave the commune code box blank. After filling in the codes of the administrative units, write the full name of commune (ward), district (town), province (city) in the blank line. For candidates under priority eligibility based on household registration, confirm the household registration duration over 18 months in Area 1 or in a specially difficult commune during high school by marking the corresponding box.
Section 6: Write the school name and address to the district (town), province (city) of the school in the dotted line. Fill the province code for the school location in the first two boxes, and the school code in the next three boxes (according to the Education Department's regulation. If the school code has 1 digit, fill the first two boxes with 0; if 2 digits, fill the first box with 0). For police or military candidates, the province code corresponds to the military location and the school code is 900. For candidates studying abroad, fill in the corresponding province code based on household registration in Vietnam and the school code is 800. Class name: write the class 12 where students are studying (e.g., 12A1, 12A2,...); for free candidates, write “TDO.”
Section 7: Clearly specify phone number, email. Candidates wanting to adjust their online admission registration must register their personal mobile number to receive a one-time password (OTP) via SMS, ensuring security for online admission registration.
Section 8: Candidates must specify contact details: full name; phone number; address (house number), hamlet (street, alley), commune (ward), district (town), province (city). This is also the address to receive the Admission Notice if selected.
Section 9: Candidates wishing to use exam results for university or Preschool Education college admission should mark (X) in the adjacent box.
Section 10: Candidates must mark (X) in one of the two boxes to specify clearly if they are following the High School program or the Continuing Education program.
Section 11: Free candidates must mark (X) in one of the two boxes to distinguish between those who have not graduated from High School and those who have graduated (at the exam time).
Section 12: Candidates must write the name and code of the Exam Council as regulated by the Ministry of Education and Training in the corresponding position.
Section 13: Current 12th-grade students must submit their Registration Form at their school. Other groups submit at locations designated by the Education Department. Exam registration unit codes follow the instructions of the registration receiving unit.
Section 14: Current 12th-grade students (non-graduates) must register the exam subjects in section a. Free candidates can choose the entire set of subjects (at section a) or some specific ones (at section b) as per their intentions. Only one combination exam (Natural Sciences or Social Sciences) is allowed. Free candidates can select specific subjects within one combination exam. Free candidates wanting to preserve old exam scores must record those scores in Section 16, but still can register for new exams for university, college admission. Continuing Education students may choose a foreign language exam if desired.
To select exam subjects: Candidates must mark (X) in the corresponding exam boxes. For Foreign Language, candidates must enter the specific language code as follows: N1 - English, N2 - Russian, N3 - French, N4 - Chinese, N5 - German, N6 - Japanese.
Section 15: Candidates wishing to be exempt from the foreign language exam or use certificates for university admission must specify the certificate type or if they are national team members for international Olympic competitions in the foreign language, as per Ministry of Education and Training regulations. If the certificate has an exam score, the candidate must fill in the score.

Exam score (If the certificate has an exam score, fill it in here): 450
Section 16: Candidates from previous years who want to preserve eligible exam scores must record those scores in the corresponding boxes. Those wishing to use results for university, college admission can still register for new exams.
Section 17: Candidates must determine their priority group, recording the correct priority code as per university and Preschool Education college admission regulations and guidelines. False declarations will incur penalties according to current regulations. Priority candidates must submit valid documents when enrolling.
Section 18: Candidates for university, college admission must fill in the code of their region as follows: Region 1 (KV1) enter 1, Rural Region 2 (KV2-NT) enter 2NT, Region 2 (KV2) enter 2, Region 3 (KV3) enter 3. The priority region follows where the candidate studied the longest time during their high school years. If studying each year at different schools or half time at each, the priority follows the graduation place. For priority based on household registration, record according to university and Preschool Education college admission regulations and Ministry of Education and Training guidelines. The registration software automatically determines the priority region when candidates fill in Sections 5 and 6; discrepancies require checking filled information.
Section 19: Follow the Registration Form instructions.
Section 20: Candidates aiming for results to apply for bridging courses (Preschool Education college) or university must mark (X) in the corresponding box for already graduated levels: Intermediate (TC), College (CD), or University (DH).
Section 21: This section is for candidates wishing for university, college admission. Candidates must thoroughly research admission information from desired schools and fill in the correct information for school code, field/group code, field/group name, and the subject combination. Incorrect entries will not be processed by the admission software.
Note:
- Free candidates include: those who have completed the high school program but haven't graduated, or have graduated high school/middle school and wish to use results for admission registration.- Candidates must fill in all sections clearly, neatly, without corrections or erasures.- Numbers must be Arabic numerals (0, 1, 2, 3,...), not Roman numerals (I, V, X,...).
Annotation:
(1) "University" for university-level programs;
(2) "College" for Preschool Education college-level programs;
(3) Those who completed the high school program but didn't graduate;
(4) Those who graduated high school, or middle school using the results for admission registration.
