1. Guide to declare the background of an applicant to the Communist Party of Vietnam

01. Full name in use: Write the full name, middle name and the name stated in the identity card or citizen's identity card, in capital letters, for example: NGUYEN VAN HUNG.

02. Male and female: As a male, cross the word "female", as a female, cross the word "male".

03. Full name of birth: Write the full name, middle name and first name stated in the birth certificate.

04. Aliases: Write the aliases used (if any).

05. Date of birth... month... year ...: Write the exact date of birth in the birth certificate.

06. Place of birth: Specify the commune (ward, town), district (district, town, city directly under the city), province (central city) where the birth certificate is issued in Vietnam according to the current name of the State administrative system.

07. Native place: Record by hometown in Vietnam in the identity card or citizen's identity card (if there is a change of administrative place, the former and present place); In special cases, it is possible to record by hometown of the mother or her breadwinner at an early age (if the father or mother is unknown). Write the address as stated in section 06.

08. Place of residence:

- Permanent residence: Write the permanent residence registration address in Vietnam (village, commune, district, province, city; house number, street, ward, town, district, city).

- Place of temporary residence: Where the temporary residence is located, the address of such temporary residence shall be written.

09. Ethnicity: Write the name of the original ethnic group such as Kinh, Thai, Tay, Nung, Muong... (if it is a foreigner, specify the nationality and ethnicity of the foreign parent).

10. Religion: By any religion, specify (for example, Buddhism, Catholicism, Islam, Cao Dai religion, Hoa Hao religion, etc., including religious dignitaries - if any), if not by any religion, write "no".

11. Current occupation: Specify the main job in Vietnam under the labor contract or decide, recruit, assign, appoint...

For example: workers, farmers, civil servants, officials, foreign doctors, staff, writers, journalists, business owners, students, students or unemployed...

12. Current qualifications:

- General education: Specify which grades have been completed, or graduated 10 years, 12 years, general or supplementary education. For example: 8/10 high school, 9/12 supplement; 12-year high school graduation.

- Vocational education (including professional secondary education and vocational training): Record according to the certificates and diplomas issued. For example: Welding Technician of the 3rd level, Veterinary Intermediate...

- Undergraduate and postgraduate education (including college, university, master and doctoral degrees): Record according to the diplomas issued in Vietnam, which majors, forms of training (formal, in-service, specialized, distance training). For example: College of Education, University of Agriculture, Bachelor of Laws, Mechanical Engineer, Doctor of Foreign Studies; Master of Economics, Doctor of Mathematics, Doctor of Science ...

If you have more than one degree, write it all down.

- Academic title: Enrolled in titles assigned by the State (Professor, Associate Professor).

- Political theory: Record according to the highest certificates and diplomas issued such as primary, intermediate, advanced, bachelor's degree; focused or unfocused study.

For those cases where they have studied political theory at training institutions outside the political school system of the Communist Party of Vietnam  or have studied specialized and postgraduate politics at the Ho Chi Minh National Academy of Politics and Administration, they shall record the political theory level according to the certificate of political theory level issued by the competent agencies in accordance with the regulations of the Secretariat and the guidance of the Central Organization Committee.

- Foreign language: Record according to the diploma or certificate issued (for example, university English, French, Russian... or English level A...).

- Informatics: For the retraining system, record according to the certificates and certifications already issued (for example, office informatics; computer science at A, B, C, etc.); if you graduate from a university specializing in informatics, write the name of the university.

- Ethnic minority language: To speak any ethnic minority language, the name of the ethnic group shall be specified.

13. Date and place of admission to Ho Chi Minh Communist Youth Union: Specify the date and place of admission to the Union (branch, grassroots union, district, province or central agency).

14. For applicants re-engaged in the Communist Party of Vietnam:

- Date and place of admission to the Communist Party of Vietnam for the first time: Specify the date and place of admission to the Party (Party department, grassroots party, district, province or central agency).

- Date and place of the first official recognition: Specify the date and place of official recognition (party, grassroots party, district, province or central agency).

- Introducers to the Communist Party of Vietnam for the first time: Specify the full name, position and working unit of each person at the time of introduction to the Party. If the grassroots trade union executive committee or grassroots trade union group introduces, specify the name of the grassroots youth union organization and the immediate superior youth union (if the grassroots trade union executive committee introduces, the same must be stated).

15. History of self: Summarize the process from adolescence to the date of participating in social activities (such as the date of youth Union, enlistment date, the date of studying at universities, colleges, professional high schools or the date of participating in activities in economic and social organizations...). For example, the asylum seeker was born into the Party in 1985, revealing his or her own history:

From September 1991 to August 2000, he attended Primary and Secondary School in Van Ho School, Hai Ba Trung District.

18. Passed training and retraining classes: Specify which political or professional reasoning classes, according to what programs; which levels, name of school, time of study, where; regular or in-service studies; names of diplomas or certificates issued in Vietnam.

19. Going abroad: Specify the time from month to year, going abroad; which agencies, units and organizations decide (only record cases of studying, cooperative labor, working...from 3 months or more).

20. Commendation: Specify the month, year, form of commendation (from commendation), level of decision; titles awarded by the State: Labor Hero, Armed Forces Hero, People's Artist, Elite Artist, People's Teacher, Elite Teacher, People's Physician...

21. Discipline: Specify the month, year, reason for the mistake, form of discipline (discipline of the party, government, unions with reprimand or more). Which level is decided in Vietnam?

22. Family situation in Vietnam: Specify the main family members such as:

- For grandparents, grandparents of themselves and their spouses: Specify full name, year of birth, hometown, place of residence, occupation, political history of each person according to the regulations of the Politburo and the guidance of the Central Organization Board on internal political protection.

- Father, mother (or foster parent); father, mother-in-law (or father, mother-in-law); wife (or husband).

23. Self-review: Bearing in mind the main advantages and disadvantages in terms of political qualities, lifestyle ethics, working capacity and public relations; the credibility of the masses and party members in the working unit, how to work for themselves?

24.Declare and sign: Specify "I assure that I have fully, clearly, honestly and responsibly declared before the Communist Party  of Vietnam on the contents declared in the curriculum vitae"; date, sign and write full name.

Note: Branches and grassroots committees have not commented on, not yet certified, signed, stamped on the resume but only affixed a stamp on all pages and photos in the resume of the applicant to the Communist Party of Vietnam; send a written request for verification or send a party member to verify the resume.

No person may be appointed to the Communist Party of Vietnam or a relative (father, mother, wife, husband, brother, sister, natural child or adopted child with legal registration) of a person to the Party to conduct background checks.

25. Comments of the commissioner and the party organization to verify the background of the applicant to the Communist Party of Vietnam:

a) Comments of the branch or tribal committee; of the standing committee or of the party executive committee of the organization where the moderation is conducted

Write the necessary contents of the resume of the applicant to the Communist Party of Vietnam requested by the Party committee where the applicant has entered the Party, whether or not it is correct or not sufficient with the content of the applicant to the Party declared in the resume; The collegial committee or the standing committee of the Party committee shall agree on the content recorded in the section "Comments of the committee, party organization..." at the end of the "Resume of the applicant to the Party". The person on behalf of the authorization level certifies, signs, clearly specifies the position and seal of the authorization level.

b) Comments of agencies or organizations or of the standing committees on the basis of (if any)

Write the necessary contents of the resume of the applicant to the Communist Party of Vietnam as requested by the Party Committee where the applicant to the Party is right, or not right or not enough with the content of the applicant to the Party declared in the resume; The standing collective of the committee or the leadership of the committee organizer unifies the content recorded in the section "Comments of the committee, party organization..." at the end of the "Resume of the applicant to the Party".

The person acting on behalf of the standing committee or the leader of the organizing committee shall certify and sign and specify the position and seal of the standing committee or the organizing committee.

26. Comments of the commissar or his/her staff (where there is no commissar): After having the results of the background verification of the applicant to the Communist Party of Vietnam, does the commissar, secretary or deputy secretary clearly state that the background statement is true? At what point is that not true? Is there a problem with the current history of politics and politics? What are the views, stances, moral qualities, lifestyle and public relations... of those applying to the Party?

27. Certification of grassroots committees: The grassroots committees shall re-evaluate the results of verification, verification and clarification of unclear or questionable issues in the background of applicants to the Party.

If the applicant for admission to the Party has issues that need to be considered politically (including the current political and political history), he/she must be concluded by the competent committee in accordance with the regulations of the Politburo, the guidelines of the Central Organization Board (before considering and deciding on admission to the Party).

After the grassroots committee is considered and concluded, the secretary of the committee shall clearly state: "The certificate of background of the masses... declared at the party ministry (or ministry)... is true; there are no (or any) problems in the current political and political history of the people entering the Communist Party of Vietnam in accordance with the regulations of the Politburo; the masses... are sufficient (or not eligible) to consider admission to the Party"; clearly state the date, month, year, position, signature, full name and seal of the grassroots committee.

In case the grassroots commissioner does not have the seal, the immediate superior commissioner shall certify the signature of the secretary of the grassroots commissioner, clearly write the position, sign and seal of the commissioner.

2. Verification and verification of the background of applicants to the Communist Party of Vietnam

- If a person entering the Communist Party of Vietnam has a close relative (Section 22) who is a party member who has fully, clearly and truthfully testified in his/her curriculum vitae as prescribed, he/she shall not be required to verify the political history but shall be required to check and verify the current politics and the observance of the Party's guidelines, policies and laws of the State and the obligations of citizens of the place of residence.

- If the relatives of the participants in the Communist Party of Vietnam are not party members, they shall verify and verify the current political history and the observance of the Party's guidelines, policies and laws of the State and the obligations of the citizens of the place of residence.
