1. In what case can the pharmacy practice certificate be re-issued in Vietnam?

Pursuant to Clause 2, Article 12 of the Law on Pharmacy 2016, the re-issuance of the pharmacy practice certificate is as follows:

The pharmacy practice certificate shall be reissued if it is lost or damaged.

2. Application for re-issuance of the pharmacy practice certificate in Vietnam

In Article 25 of the Law on Pharmacy 2016, the application for re-grant of a pharmacy practice certificate is as follows:

 - An application form having the applicant’s picture which is taken within the last 06 months.

- A Copy of the issued pharmacy practice certificate; the applicant’s commitment is required if the pharmacy practice certificate is lost.

3. Procedures for re-issuance of pharmacy practice certificates in Vietnam

Procedures for re-issuance of a pharmacy practice certificate in Article 27 of the Law on Pharmacy 2016 are as follows:

Step 1: Submit your application

The application for the pharmacy practice certificate shall submit the application to the issuing authority.

Step 2: Process the application and return the results

Within 20 days from the day on which the satisfactory application for issuance of the certificate is received;

After 10 days for applications for reissuance or adjustment of the certificate, the head of the issuing authority shall issue the pharmacy practice certificate, or provide explanation if the application is rejected.

Step 3: Issuance of a pharmacy practice certificate

The time limit for issuing the pharmacy practice certificate specified in Clause 8 Article 24 of Law on Pharmacy 2016 is 05 working days from the day on which the application is received.

4. Competent entities to reissue pharmacy practice certificates in Vietnam

Article 23 of the Law on Pharmacy 2016 stipulates the competent entities to reissue as follows:

Directors of Services of Health of provinces in Vietnam shall issue, reissue, adjust, revoke pharmacy practice certificates that do not require an examination.

The Director of each Service of Health of Vietnam shall establish an advisory council participated by representatives of a pharmacy association to advise the Director of the Service of Health about the issuance, reissuance, and revocation of pharmacy practice certificates.
