issued along with this Decision is an Appendix comprising 14 Forms for the public to proceed with procedures to receive support due to Covid-19, specifically including:
- Form No. 1: List of employees temporarily suspended from performing labor contracts, taking unpaid leave;
- Form No. 2: Request for support (For business households with tax declared revenue below 100 million VND/year);
- Form No. 3: Request for support (For employees terminated from labor contracts, work contracts but not eligible for unemployment benefits);
- Form No. 4: Request for support (For employees without labor contracts who have lost their jobs);
- Form No. 5: List of reviewed beneficiaries eligible for support due to the COVID-19 pandemic (district level);
- Form No. 6: List of people with meritorious services who are facing difficulties due to the COVID-19 pandemic (provincial level);
- Form No. 7: List of social protection beneficiaries receiving monthly social benefits eligible for support due to the impact of the COVID-19 pandemic;
- Form No. 8: List of individuals from poor households eligible for support policy;
- Form No. 9: List of individuals from near-poor households eligible for support policy;
- Form No. 10: Summary table of the list of poor and near-poor households eligible for support policy;
- Form No. 11: Request for a loan to support wage payment for employees during work stoppage;
- Form No. 12: List of employees suspended from work due to the impact of the COVID-19 pandemic (for enterprises);
- Form No. 13: List of employers eligible for loans to pay wages during work stoppage;
- Form No. 14: List of employees under work stoppage (provincial level).
