Conversion from CMND to CCCD, What Do Citizens Need to Do?

To replace the People's Identity Card (CMND) with the Citizen Identity Card (CCCD), citizens are required to follow the specific procedures for issuing a new CCCD as outlined below.

According to the provisions of Article 12 of Circular 07/2016/TT-BCA, as amended and supplemented at Clauses 2 and 3, Article 1 of Circular 40/2019/TT-BCA, the procedure for issuing a new CCCD card is carried out as follows:

Step 1: Fill in the CCCD declaration form

Citizens fill in the CCCD declaration form. In case citizens provide information using the CCCD declaration form on the online public service portal, they select the date, month, and year for the CCCD card issuance procedure and send the declaration form to the CCCD management agency where they register for permanent residence through the online public service portal. For localities that receive CCCD card issuance applications at the reception and result return departments at the district level or at the provincial public administrative service center, after receiving the applications, the reception officer will forward the applications to the competent CCCD management agency.

Step 2: Receive the application

The officer receiving the CCCD card issuance application at the CCCD management agency will check and verify the information of the citizen in the CCCD declaration form (in case citizens provide information online, it will be received through barcode readers or information collection devices into the system) with the information in the national population database connected with the CCCD database to accurately identify the person needing the card and confirm the information about the citizen.

In the event that the national population database has not been operational, citizens are required to present their household registration book. If the information on the household registration book is incomplete or inconsistent with the information provided in the CCCD declaration form, citizens must present their birth certificate, old CMND, or other legal documents regarding the information needed to be recorded in the CCCD declaration form.

In case citizens apply for a CCCD card at the district-level CCCD management authority needing amendments in the household registration book, which falls under the jurisdiction of the district police, and citizens present all valid documents as per the residency laws, the application will be received to amend the household registration book along with the CCCD card issuance application. The receiving officer will forward the amended household registration book application to the registration and residence management department for amendment. After the amendment, the adjusted household registration book will be returned to the receiving officer for the CCCD card issuance application.

Note, for citizens transitioning from a 9-digit CMND or 12-digit CMND to a CCCD card, the officer receiving the application will collect, file, and handle the CMND as per regulations.

Step 3: Take photos, collect fingerprints and print the Information Receipt Form

The CCCD management agency officer takes photos, collects fingerprints, and identifies characteristics of the person applying for a CCCD card to print on the CCCD information receipt form and the CCCD card as per regulations.

Details:- The citizen's portrait photo must be a headshot, with bare head, clearly showing the face and both ears, not wearing glasses; the attire and demeanor must be serious, formal, and without specialized uniforms when taking the CCCD card photo; for citizens who follow a religion or ethnic customs, it is permissible to wear religious or ethnic attire, and if there is a headscarf, it is allowed to be worn for the CCCD card photo but must ensure the face is clear;- The CCCD management agency officer will collect fingerprints through a fingerprint collection device; for fingers that are missing, bent, deformed, or cannot collect fingerprints, specific details will be recorded in the corresponding position of that finger.

Step 4: Issue an appointment slip for CCCD card collection

The CCCD management agency officer will issue an appointment slip to the person applying for the CCCD card. If the application and procedures are not complete as per regulations, the citizen will be guided to complete them for CCCD card issuance.

Step 5: Receive the CCCD card

The agency where the application is received will issue the Confirmation of CMND Number, return the CCCD card, and the household registration book (if any) according to the time and location specified in the appointment slip. The CCCD card collection location is the place where the card issuance procedure is conducted.

In cases where citizens request the card to be delivered to another location, they must specify the address for card delivery on the CCCD declaration form. The agency receiving the application will deliver the CCCD card to the specified address as requested by the citizen, ensuring timely delivery, and citizens will have to pay the delivery service fee as per regulations.

Note, individuals suffering from mental illness or other illnesses that impair their ability to perceive or control their behavior must have a legal representative present to complete the procedure.
