Issued together with Circular No. 15/2014/TT-BCA of the Ministry of Public Security of Vietnam are 16 forms used in vehicle registration, including:

Form 01: The written notification of a vehicle used as a gift or a transferred vehicle

Form 02: The declaration of vehicle registration

Form 03: The registration of title transfer and vehicle relocation

Form 04: The declaration of title transfer and vehicle relocation

Form 05: The declaration on revoking the certificate of license plate and vehicle registration

Form 06: The certificate of the revocation of license plate and vehicle registration

Form 07: The issuance of inspection badges

Form 08: The supervisory book of temporarily imported and re-exported automobiles operating in special economic – commercial zones

Form 09: The certificate of motorbike registration

Form 10: The certificate of automobile registration

Form 11: The certificate of trailer and semi-trailer registration

Form 12: The certificate of temporary vehicle registration

Form 13: The certificate of tractor registration

Form 14: The certificate of motorbike registration applied to those used at special economic – commercial zones or international economic bordergate zones as regulated by the Government

Form 15: The certificate of automobile registration applied to those used at special economic – commercial zones or international economic bordergate zones as regulated by the Government

Form 16: The certificate of trailer and semi-trailer registration applied to those used at special economic – commercial zones or international economic bordergate zones as regulated by the Government

Note: Those built with the similar specifications to a single vehicle must adopt the registration form of the latter.
