In order to ensure that the entitlement to health examination and treatment is not affected in the event of losing the health insurance card, everyone should pay attention to comply with the following regulations to be reissued a lost health insurance card.
Article 18 of the Health Insurance Law 2008 amended in 2014 stipulates that health insurance cards can be reissued in case of loss, and individuals who lose their health insurance cards must submit an application for reissuance.
1. Composition of the reissuance application file
- Declaration form for participation, adjustment of Social Insurance information, Health Insurance (Form TK1-TS)
- In case the employee is entitled to higher Health Insurance benefits, supplementary proof documents, if any, according to Annex 03
- Information list for the employer (Form D01-TS)
2. Place for submitting the reissuance application file for the health insurance card
Clause 3 Article 3 of Decision 595/QD-BHXH stipulates:
- Participants in health insurance collected by the district social insurance agency must submit the application at the district social insurance agency.
- Participants in health insurance collected by the provincial social insurance agency must submit the application at the provincial social insurance agency.
3. Time limit for resolution
Clause 2 Article 30 of Decision 595/QD-BHXH stipulates that the time limit for resolving the reissuance application file for the health insurance card is 02 working days from the date of receiving the complete application file as prescribed.
4. Cases of lost cards but still entitled to medical examination and treatment expenses
a. Case with an appointment letter for reissuing the health insurance card
Clause 3 Article 15 of Decree 146/2018/ND-CP stipulates that participants in health insurance, during the period awaiting reissuance or replacement of the health insurance card, must present the appointment letter for reissuing or replacing the health insurance card issued by the social insurance agency or the authorized organization or individual receiving the reissuance application file, according to Form No. 4 of the Annex issued with this Decree, along with one type of document proving the individual's identity.
b. Case entitled to direct payment by the social insurance agency
Point c Clause 1 Article 4 of Circular 09/2019/TT-BYT stipulates that participants in health insurance are entitled to direct payment for medical examination and treatment expenses in case the patient cannot present the health insurance card before being discharged or transferred on the same day due to losing the health insurance card but has not yet been reissued.
