What does the Dossier for Application for Recognition and Issuance of National Occupational Skills Certificate Include?

Circular 38/2015/TT-BLDTBXH stipulates that the dossier requesting recognition and issuance of the national vocational skill certificate for workers participating in the evaluation and meeting the requirements for the national vocational skill certificate includes:

- A written request for recognition and issuance of the vocational skill certificate by the organization assessing the vocational skill according to Form 01, Appendix 03 issued together with this Circular;- The list requesting recognition and issuance of the national vocational skill certificate for those who meet the requirements, according to Form 02, Appendix 03 issued together with this Circular, and attached to each person on the list is one (01) colored photo with a white background, size 03 x 04 cm, ID card style of that person;- The decision of the head of the vocational skill assessment organization to recognize the assessment results of the examination board with an attached summary of the results of the knowledge test and the practical test of the workers participating in the evaluation and issuance of the national vocational skill certificate, prepared by the examination board with the full signatures of all members of the examination board confirming the assessment results.

Furthermore, the Circular also stipulates the dossier requesting recognition and issuance of the vocational skill certificate for those who win medals at the world skills competition, ASEAN skills competition, and those who win first, second, and third prizes at the national skills competition, to be conducted by the skills competition organizing committee according to Form 01, Appendix 04 issued together with this Circular.
