Unified Management of Forms Used in Issuance and Administration of Citizen Identification Cards

This is a regulation mentioned in Circular 41/2019/TT-BCA amending and supplementing a number of articles of Circular 66/2015/TT-BCA on the forms used in the issuance and management of Citizen Identification Cards, citizen identification archives, and the National Database on Population.

To be specific:, the management of forms used for the issuance and management of Citizen Identity Cards, citizen archives, and the National Population Database is stipulated in Circular 66/2015/TT-BCA, amended and supplemented in Clause 2, Article 1 of Circular 41/2019/TT-BCA as follows:

- The Ministry of Public Security shall uniformly manage the forms issued with this Circular; print and distribute form CC11 to the police departments of units and localities.- The Department of Administrative Management on Social Order shall print, manage, and distribute forms CC01 and CC13 for use at the National Citizen Identity Center.- Provincial and centrally-governed city police shall organize the printing, management, and distribution of forms CC01, CC02, CC08, CC12, CC13, DC01, and DC02 to the police departments of units and localities under their management.- Forms CC02, CC03, CC04, CC05, CC06, CC07, CC09, and CC10 shall be printed directly from the computer when processing the issuance, renewal, and reissuance of Citizen Identity Cards. The printed forms will include information extracted and compiled from the citizen identity database according to the storage scope of each unit.- Forms CC01, CC02, CC04, CC05, CC06, CC07, CC08, CC09, CC10, CC12, CC13, DC01, and DC02 shall be printed on paper size 210 mm x 297 mm (A4). Form CC03 shall be printed on paper size 148 mm x 210 mm (A5). Forms CC01 and CC08 shall be printed on both sides. Form CC11 measures 250 mm x 330 mm, printed on Kraft paper with a weight of 250g/m2.

The forms stipulated in this Circular shall be printed in black ink on white paper. Form CC11 shall be printed in black ink on yellow-brown paper. When printing, the content of the forms must not be changed; the police departments of units and localities are responsible for managing the printing and issuance of forms and maintaining tracking records.
