Today, I self-assess my conduct regarding the following incidents:

- Violation committed: I have violated the population policy and family planning regulations by having a third child.

- Reason for the violation: I have implemented several family planning measures, such as using an intrauterine device (IUD), but I still became unintentionally pregnant. Due to my insecure health condition, which could endanger my life, I decided not to have an abortion. Hence, I have given birth to a third child.

- Regarding the disciplinary measures: According to Clause 1, Article 5 Regulations on disciplinary measures for officials and public employees of the Ministry of Finance who violate the population policy and family planning regulations issued in conjunction with Decision 1531/QD-BTC dated June 23, 2013 by the Minister of Finance, the disciplinary measure for me is as follows: The disciplinary measure "Reprimand" applies to officials and public employees having a third child.

- Based on the issued disciplinary regulations, I self-impose the disciplinary measure: Reprimand.

I pledge not to repeat the violation of the population policy and family planning regulations.
