Regulations on Printing Templates in the Issuance and Management of Citizen Identity Cards

The Ministry of Public Security has issued Circular 66/2015/TT-BCA on the regulations for the forms used in the issuance and management of citizen identification cards, citizen identification archives, and the national database on population. For more details, please visit this link.

Forms for the issuance and management of Citizen ID cards must be printed according to the following regulations:

- Forms CC02, CC03, CC04, CC05, CC06, CC07, CC09, and CC10 are to be printed directly from the computer when processing the issuance, replacement, or reissuance of Citizen ID cards. The printed forms already include information extracted and compiled from the Citizen ID Database within the storage scope of each unit.- Forms CC01, CC02, CC04, CC05, CC06, CC08, CC09, CC10, DC01, and DC02 are to be printed on 210 mm x 297 mm (A4) paper; specifically, form CC02 has a weight of 120 g/m2.- Forms CC03 and CC07 are to be printed on 148 mm x 210 mm (A5) paper.- Forms CC01 and CC08 are to be printed on both sides.- Form CC11 has dimensions of 250 mm x 330 mm and is printed on Kraft paper with a weight of 250 g/m2.- The forms are to be printed in black ink on white paper. Form CC11 is specifically printed in black ink on brown-yellow paper.

When printing, the content of the forms must not be altered; the Public Security units and localities are responsible for managing the printing and distribution of forms and for maintaining tracking records.
