Circular 43/2018/TT-BYT has been issued by the Ministry of Health providing Guidelines for the Prevention of Medical Incidents in Medical Examination and Treatment Facilities.
According to Circular 43, when a medical incident occurs, the healthcare facility must report and record this incident. Specifically, the compilation of medical incident reports is carried out as follows:
1. At medical examination and treatment facilities:
- Compile medical incident reports and send them to the management agency every 6 months.- The content of the compiled report includes:- The number of mandatory and voluntary medical incident reports- Frequency of each type of incident- Results of root cause analysis- Proposed and implemented preventive solutions and recommendations
2. At the Department of Health:
- The Medical Practice Department of the Department of Health compiles medical incident reports from affiliated medical examination and treatment facilities, listing medical incidents according to the severity and frequency of occurrence.- Quarterly, compile recommendations and report to the Ministry of Health.
3. At the Ministry of Health:
- The Department of Medical Examination and Treatment Administration compiles periodic and ad-hoc reports from the medical examination and treatment facilities under the Ministry of Health and the compiled reports from the Departments of Health.- Quarterly, analyze national medical incident statistical reports and issue general prevention recommendations for medical incidents to healthcare facilities nationwide.
