Official Dispatch 5233: Responsibility for Retrieving Sample Forms of Certification for Social Insurance Leave

Vietnam Social Security has requested the Social Security Offices of provinces, centrally-run cities; the Social Security Office of the Ministry of National Defense, and the Social Security Office of People’s Police to organize the implementation:

- Recovery of unused certificate blanks issued by the Social Security Office that are still stored at medical examination and treatment facilities; stub books; invalid or damaged blanks to store or destroy according to regulations, to be completed before January 31, 2019, and simultaneously notify local medical examination and treatment facilities from January 1, 2019, not to use the certificate blanks issued by the Social Security Office to issue certificates for employees when going for medical examination and treatment; such documents using these certificate blanks will have no legal value for the payment of Social Security policies according to regulations.

- Before December 31, 2018, guide medical examination and treatment facilities to register and publicly post on the electronic portal of Vietnam Social Security the contents prescribed in Clause 2, Article 23; Points c, d, Clause 5, Article 26 of Circular 56/2017/TT-BYT including:

- Operating license, scope of certificate issuance;- Sample seal of the medical examination and treatment facility as a legal entity according to the form prescribed in Appendix 08 of Circular 56;- Sample seal of the medical examination and treatment facility and the signature of authorized practitioners to sign certificates for medical examination and treatment facilities that are not legal entities according to the form prescribed in Appendix 08 of Circular 56;- Power of attorney for individuals signing on behalf of the head of the unit and when changing information such as: Increase or decrease in the number of practitioners assigned to sign certificates at medical examination and treatment facilities that are not legal entities; change of sample seal; reassignment of individuals authorized to sign on behalf of the head of the unit, change in the scope of certificate issuance of licensed medical examination and treatment facilities.

Additionally, Vietnam Social Security also requires the Information Technology Center of Vietnam Social Security to develop a section “Medical Examination and Treatment Facilities Issued Certificates” on the Vietnam Social Security electronic portal to receive and manage information as prescribed.
