Instruction on health insurance card when removing physical household registration book in Vietnam is guided in Decision 62/QD-BYT, effective from January 12, 2023.

1. Instruction on health insurance card when removing physical household registration book in Vietnam

Step 1: For participants of health insurance for the first time, write the declaration of participation, adjust health insurance information, and form No. 2 or Form No. 3 (for participants of household health insurance) issued together with Decree 146/2018/ND-CP has been amended and supplemented in Decree 104/2022/ND-CP for the following groups of subjects:

- Employers make a list of employees participating in health insurance as prescribed in Article 1 of Decree 146/2018/ND-CP

- The educational institution and vocational training institution shall have to make a list of participants in health insurance under their management according to the provisions of Clause 15, Article 3, Clause 3, Article 4 of Decree 146/2018/ND-CP.

- Units under the Ministry of National Defense and the Ministry of Public Security are responsible for making a list of participants in health insurance under their management as prescribed in Clause 1, Article 1; Clause 13, Article 3 and Article 6 of Decree 146/2018/ND-CP and under the guidance of the Ministry of National Defense and the Ministry of Public Security.

- For a person who has donated body parts as prescribed by law, the social insurance agency shall base its decision on the hospital discharge paper issued by the medical facility where the organ donor is located to issue a health insurance card.

- Commune People's Committees are responsible for making a list of subjects specified in Article 2; Clauses 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 16, 17 Article 3; Clauses 1, 2, 4, Article 4 and Article 5 of Decree 146/2018/ND-CP.

Step 2: The participants of health insurance submit to the social insurance agency and wait for the signature (in the box of the applicant) in the receipt of the application and an appointment to return the results of the re-issuance of the health insurance card according to Form No. 4 of the Appendix issued together with Decree 146/2018/ND-CP, as amended and supplemented in Decree 104/2022/ND-CP.

Step 3: Officers of the one-stop section of the provincial and district social insurance agencies check and receive the declaration of participation and adjustment of health insurance information; record the receipt of the application and the appointment to return the results of the issuance of the health insurance card according to Form No. 4 of Appendix promulgated together with Decree 146/2018/ND-CP, which was amended and supplemented in Decree 104/2022/ND-CP and signed (in the box of the person receiving the application and giving the Form 4 to the health insurance participant to sign in the box of the person submitting the application).

- Within 10 (ten) working days from the day on which the complete application is received, the health insurance organization must transfer the health insurance card to the agency or organization managing the subject or to the insured person.

2. Application for making a health insurance card when removing physical household registration book in Vietnam

Components of application for the issuance of a health insurance card include:

- Declaration of participation and adjustment of health insurance information (of organizations and individuals for participants in health insurance) issued by the Vietnam Social Insurance and the list of participants in health insurance of organizations and individuals made according to Form No. 2 of the Appendix issued together with Decree 146/2018/ND-CP.

- The declaration of participation and adjustment of health insurance information of the participants of health insurance by household and the list of household members participating in health insurance made according to Form No. 3 of the Appendix promulgated together with Decree 146/2018/ND-CP, which was amended and supplemented in Decree 104/2022/ND-CP.

3. Conditions for issuance of health insurance cards when removing physical household registration book in Vietnam

- Health insurance cards are issued to participants of health insurance and serve as the basis for enjoying health insurance benefits in accordance with the Law on Health Insurance.

- Only one health insurance card per person.
