The General Director of Vietnam Social Security issued Decision 58/QD-BHXH on January 22, 2025, regarding the Form for Notification of Administrative Procedure Results; the result codes for administrative procedures under the authority of Vietnam Social Security.

Guidelines on the form for Notification No. 01-TB for providing results of social insurance, health insurance, and unemployment insurance procedure resolution in Vietnam

Issued together with Decision 58/QD-BHXH on January 22, 2025, is the Notification form for results of administrative procedures common for scenarios where the result is not a document or specific form, but rather financial assistance or a sum disbursed to the beneficiary, or the result is documented in a social insurance agency's database (see attached Form No. 01-TB).

Instructions for filling out Form No. 01-TB applied in responding to results of administrative procedures in social insurance, health insurance, unemployment insurance:

(1): File number

(2): Record the name of the organization for organization cases, and the name of the individual for individual cases.

(3): Record according to individual identification number or the unit code managed by the Social Insurance agency.

(4): Name of the social insurance agency handling the matter.

(5): Record the date the file was received.

(6): Record file number.

(7): Name of the social insurance agency handling the matter.

(8): Phone number of the social insurance agency handling the matter.

Form No. 01-TB records the outcome rendered to individuals and entities; simultaneously, it is synchronized with the National Public Service Portal.

Form No. 01-TB applies to yielding administrative procedure results in the fields of social insurance, health insurance, unemployment insurance for cases such as:

- Field of Revenue Management - book, card: Notification on adjusting contributions for social insurance, health insurance, unemployment insurance, occupational accident and disease insurance, and back-payment for employees receiving a salary increase.

- Field of Social Insurance Policy Implementation: Notification on resolving benefits for maternity leave for employees who resign before the childbirth, adoption, or foster care.

- Field of Health Insurance Policy Implementation: Notification on resolving direct payment for medical examination and treatment costs for health insurance participants.

- Field of disbursing social insurance benefits:

+ Notification on cases where beneficiaries of social insurance benefits request to switch from receiving cash to a personal account or vice versa, or change personal information.

+ Notification on cases of authorized substitutes claiming social insurance benefits, unemployment aid, or claim deferred pension, social insurance benefits of the deceased beneficiary with receivable sources.

Other occurrences where the administrative procedure results, public service cannot be expressed in writing or form.

Additionally, the codification of result codes for administrative procedures pertaining to social insurance books, health insurance cards, and Notification of Administrative Procedure Results (Form No. 01-TB) is as follows:

- Social Insurance Book: KQ.G24.000002;

- Health Insurance Card: KQ.G24.000004;

- Notification of Administrative Procedure Result (Form No. 01-TB): KQ.G24.000051.

The Chief of Office of Vietnam Social Security, Director of the Information Technology Center, Heads of affiliate units under Vietnam Social Security, Directors of Social Insurance in provinces and centrally governed cities, and relevant organizations and individuals are responsible for implementation.
