Guidelines on the Application for Monthly Living Allowance for Eminent Artisans

The dossier for receiving a monthly allowance is applicable to distinguished artisans and meritorious artisans with low incomes and difficult circumstances as stipulated in Clause 1, Article 7 of Decree 109/2015/ND-CP, specifically includes:

- A declaration form by the person requesting the monthly allowance and health insurance according to form No. 01 issued together with this Decree;

- Certified copy from the original or copy (with the original for comparison) of the Decision to award the title "Distinguished Artisan" or "Meritorious Artisan" of the person requesting the monthly allowance and health insurance;

- Certified copy from the original or copy (with the original for comparison) of one of the following related documents:

+ Identity Card for the subjects stipulated in Point a, Clause 1, Article 2 of this Decree;

+ Disability Certificate for the subjects stipulated in Point b, Clause 1, Article 2 of this Decree;

+ Discharge paper (in case of inpatient treatment) or Consultation Record or Meeting Minutes of the hospital or health record book provided by healthcare institutions according to the law on medical examination and treatment (in case of outpatient treatment), which indicates treatment for diseases requiring long-term treatment for the subjects stipulated in Point c, Clause 1, Article 2 of this Decree.

Additionally, Decree 109 also stipulates the dossier for requesting funeral expenses support, includes:

- Declaration form requesting funeral expenses support by the agency, organization, or individual directly organizing the funeral according to form No. 3a or No. 3b issued together with this Decree;

- Certified copy from the original or copy (with the original for comparison) of the Death Certificate issued by the competent authority.
