The Ministry of Health recently issued Circular 43/2018/TT-BYT Guiding the prevention of medical incidents in healthcare facilities.
In the event of a medical incident at a medical examination and treatment facility, incident reporting must be carried out voluntarily or mandatorily, specifically:
- Voluntary Reporting: The individual directly causing the medical incident or the person discovering the medical incident reports it to the medical incident reception and management department of the medical examination and treatment facility.- Mandatory Reporting: The individual directly causing the medical incident or the person discovering the medical incident must report it to the department head, the medical incident reception and management department. The department head is responsible for reporting to the Leadership of the medical examination and treatment facility. The Leadership of the medical examination and treatment facility is directly responsible for reporting immediately to the managing authority of the medical examination and treatment facility.
Regarding the recording of medical incidents, it is carried out as follows:
- The Quality Management Department, Quality Management Team, or dedicated quality management staff are the receiving and managing departments for medical incidents at the medical examination and treatment facility and perform the recording of direct reports, reports via telephone using the Medical Incident Report Form for retention.- All reported medical incidents must be recorded and retained in the records or in the online medical incident reporting system.
