Guidelines for Filling Out the Citizen Identification Declaration Form (CC01)

The Ministry of Public Security has issued Circular 66/2015/TT-BCA regulating the forms used in the issuance and management of citizen identification cards, citizen identification records, and the national database on population.

Clause 2, Article 7 of Circular 66/2015/TT-BCA guides the procedure for filling out information in the Citizen Identification Declaration Form (CC01) as follows:

- Section “Full name, middle name, and first name”, “Other names”: record the full name, middle name, and first name as per the birth certificate; uppercase letters including accents. Only record the surname and other names if listed in the birth certificate.

- Section “Date of birth”: record the date of birth of the citizen being issued, renewed, or reissued the Citizen Identification Card. The day of birth recorded in 2 digits; the year of birth in 4 digits. For months from March to September, record in 1 digit; other months are recorded in 2 digits.

- Section “Gender”: if male, record as “Male”, if female, record as “Female”.

- Section “Ethnicity”, “Religion”: record the ethnicity and religion of the citizen being issued, renewed, or reissued the Citizen Identification Card as per the birth certificate or authoritative documents certifying ethnicity and religion.

- Section “Nationality”: record the nationality of the citizen being issued, renewed, or reissued the Citizen Identification Card as per the birth certificate or authoritative documents proving Vietnamese nationality.

- Section “Marital status”: record the current marital status of the citizen being issued, renewed, or reissued the Citizen Identification Card, including: single, married, or divorced.

- Section “Blood type” (if any): record based on the blood type determination test results of the citizen requesting issuance, renewal, or reissuance of the Citizen Identification Card.

- Section “Place of birth registration”: record the full administrative location at the commune, district, and provincial levels where the birth certificate was issued to the citizen. If there has been an administrative location change, record the new administrative name accordingly as per legal regulations.

- Section “Hometown”: record the full administrative location at the commune, district, and provincial levels as per the birth certificate; if no birth certificate is available, record as per the household registration book. If there has been an administrative location change, record the new administrative name accordingly as per legal regulations.

- Section “Permanent residence”: record fully and accurately as per the household registration book. For citizens who are commissioned officers in the People's Army or the People's Police residing in barracks or collective housing units, record as per the introduction letter issued by the agency or unit.

- Section “Current place of residence”: record fully, clearly, and accurately the current place of residence of the citizen, including house number, street; hamlet, village; commune/ward/town; district/town/city under province; province/city under central government.

- Section “Occupation”: specify the current occupation, if a soldier is on active duty, leave it blank. Section “Educational level”: specify the highest level of education (Doctorate, Master's, University, College, Intermediate, High School graduate, Secondary School graduate...).

- Sections 17, 18, 19, 20, 21: record fully the name, middle name, first name; nationality; Citizen Identification number or Identity Card number in the corresponding sections of the form (if any).

- Citizen’s request section:

- “Delivery by post to the citizen’s hand”: if the citizen requests postal delivery for issuance, renewal, or reissuance of the Citizen Identification Card, mark as yes and provide full address and phone number of the citizen; if not requested, mark as no;- “Issuance, renewal, reissuance of the Citizen Identification Card”: for first time issuance, mark as new issuance; for damaged, expired, or information change cases, mark as renewal; for lost cases, mark as reissuance;- “Confirmation of Identity Card number”: if the citizen requests confirmation of the 9-digit Identity Card number during issuance, renewal, or reissuance of the Citizen Identification Card, mark as yes; if not requested, mark as no.

- Section “Date....Month…....Year….”: specify the date, month, and year the citizen completes the declaration for issuance, renewal, or reissuance of the Citizen Identification Card.
