This is a notable provision stipulated in Circular 41/2019/TT-BCA Circular 41/2019/TT-BCA amending and supplementing certain articles of Circular 66/2015/TT-BCA regarding the forms used in the work of issuing, managing citizen identification cards, citizen identification archives, and the National Population Database.
Currently, the Citizen Identity Card Declaration Form CC01 is used for citizens to declare their personal information when applying for the issuance, replacement, or re-issuance of the Citizen Identity Card (including cases where citizens declare on the online public service information portal).

To be specific, the instructions for filling out the Citizen Identity Card Declaration (CC01) are provided in Circular 66/2015/TT-BCA and the amendments and supplements in Clauses 4, 5, 6, 7, 8 of Article 1 of Circular 41/2019/TT-BCA as follows:
- Section “Surname, middle name, and given name”, “Other surname and name”: fully write the surname, middle name, and given name as per the birth certificate; use uppercase letters with diacritics. Only write the other surname and name if the birth certificate includes them.- Section “Date of birth”: write the day, month, and year of birth of the citizen for whom the card is being issued, replaced, or reissued. The day of birth should have two digits; the year of birth should have four digits. For the month of birth, use one digit for months from March to September, and two digits for all other months.- Section “Gender”: if male, write “Male”; if female, write “Female”.- Section “Ethnicity”, “Religion”: write the ethnicity and religion of the citizen as stated in the birth certificate or official documents certifying ethnicity or religion by the competent authority.- Section “Nationality”: write the nationality of the citizen as stated in the birth certificate or official documents proving Vietnamese nationality by the competent authority.- Section “Marital status”: write the current marital status of the citizen, including: single, married, or divorced.- Section “Blood group” (if available): write according to the result of the blood type test of the citizen requesting the issuance, replacement, or reissuance of the Citizen Identity Card.- Section “Place of birth registration”: Write the administrative location at the commune, district, and provincial levels as per the citizen’s birth certificate. If the birth certificate does not fully specify these administrative locations, write them as stated in the birth certificate. If the administrative locations have changed, write the new administrative locations following the regulations.- Section “Hometown”: Write the administrative location at the commune, district, and provincial levels as per the birth certificate or household registration book. If these documents do not fully specify these administrative locations, write them as stated in the documents. If the administrative locations have changed, write the new administrative locations following the regulations.- Section “Citizen's request”:- “Issuance, replacement, reissuance of the Citizen Identity Card”: For initial issuance, write “new issuance”; for damage, expiration, or information changes/errors in the Citizen Identity Card, or if the citizen requests a replacement, write “replacement”; for lost cards or returning to Vietnamese nationality, write “reissuance”.- “Delivery of the Citizen Identity Card to the citizen’s address”: If the citizen requests the delivery of the card, write “yes” and provide the full address and contact phone number; if not, write “no”.
- The two-dimensional barcode section encodes all personal information of the citizen and management information on the Online Citizen Identity Card Declaration.- Section “Appointment time”: Register the specific date when the citizen will visit the card management agency to process the issuance, replacement, or reissuance of the Citizen Identity Card; section “At”: Write the name of the card management agency where the citizen registers to process.
