Decree 13/2018/ND-CP on Guidance of the Law on Access to Information was issued on January 23, 2018. The annex issued together with this Decree provides forms used in the provision of information upon request.

To be specific, the forms used in providing information upon request as stipulated in the Appendix issued together with Decree 13/2018/ND-CP include:

- Information Request Form: Form No. 01a or Form No. 01b;- Information Request Receipt Form: Form No. 02;- Information Request Resolution Form: Form No. 03;- Information Request Extension Notice Form: Form No. 04;- Information Request Denial Notice Form: Form No. 05.

In the case where the request for information is stipulated in Clauses 1 and 2, Article 7 of the Law on Access to Information, it must be accompanied by a written approval from the relevant individuals or organizations. The approval document of the relevant individuals or organizations is to be carried out according to Form No. 06 in the Appendix issued together with Decree 13/2018/ND-CP.
