Forms for applying and enforcing educational measures at commune, ward, commune-level town (Illustrative image)

According to Article 3 of Circular 42/2014/TT-BCA regulating the forms used when applying and enforcing educational measures at communes, wards, commune-level towns, and home management measures, include:

- Proposal to establish a dossier for applying educational measures at commune, ward, commune-level town;

- Proposal to apply home management measures;

- Proposal for exemption from serving the remaining time of the Decision on applying educational measures at commune, ward, commune-level town;

- Notification about the establishment of a dossier for applying educational measures at commune, ward, commune-level town;

- Notification about the continuation of management and education for the person subjected to educational measures at commune, ward, commune-level town;

- Report on verification results for the person proposed for applying educational measures at commune, ward, commune-level town/home management measures;

- Report on monitoring, educating, and assisting results for the person educated at commune, ward, commune-level town;

- Minutes of the meeting on reviewing the application of educational measures at commune, ward, commune-level town/home management measures;

- Minutes of the meeting on commenting for the person educated at commune, ward, commune-level town;

- Commitment form regarding adherence to decisions by the person educated at commune, ward, commune-level town;

- Commitment form ensuring conditions to implement home management;

- Decision on applying educational measures at commune, ward, commune-level town/home management measures;

- Decision on not applying educational measures at commune, ward, commune-level town;

- Decision to exempt from serving the remaining time of the Decision on applying educational measures at commune, ward, commune-level town;

- Decision to temporarily suspend the implementation of the Decision on applying educational measures at commune, ward, commune-level town;

- Decision to terminate the application of home management measures;

- Certificate of completion of the educational period at commune, ward, commune-level town;

- Summary of the background of the person proposed for applying educational measures at commune, ward, commune-level town/home management measures;

- Application for permission to leave the residence;

- Summons.
