Dossier for Recommendation of the Certificate of Merit from the Minister of Science and Technology applicable from December 10, 2019

at Article 9 of Circular 16/2019/TT-BKHCN, it is stipulated that the dossier proposing the Minister of Science and Technology's Certificate of Merit includes 01 set (original copy) to be sent directly or by mail to the Ministry of Science and Technology (through the Department of Emulation and Commendation), specifically:

First, for commendation of outstanding achievements in the field of science and technology:

- Proposal document from the Head of the agency, accompanied by a summary list of achievements of the collective or individual proposed for commendation, specifically:

- Proposal document from the Ministry, ministerial-level agencies, agencies under the Government of Vietnam for collectives and individuals under such entities;- Proposal document (or acknowledgment in the achievement report) from the People's Committee of the province, centrally-run city for the Department of Science and Technology and the Director of the Department of Science and Technology; the Department of Science and Technology for collectives and individuals under this department;- Proposal document from the Ministry, ministerial-level agencies, agencies under the Government of Vietnam or the Chairman of the People's Committee of the province, centrally-run city (based on the proposal of subordinate state agencies) for collectives and individuals not belonging to the above-mentioned cases.

- Achievement report of the collective or individual proposed for commendation (according to Form 02 issued with this Circular).

Second, for commendation by topic:

- Proposal document from the presiding unit organizing the topic;

- Document from the Head of the proposing agency or organization;

- Achievement report of the collective or individual proposed for commendation (according to Form 03 issued with this Circular).

Third, for commendation of unexpected achievements in the field of science and technology:

- Document from the Head of the competent agency proposing for commendation;

- Achievement report of the collective or individual proposed for commendation, clearly stating the content, extent, and scope of the impact of the achieved results (according to Form 04 issued with this Circular).
