- Official Dispatch from the General Department, Department, or Sub-department sent to the Inspectorate of the Ministry of Industry and Trade requesting the issuance of the Card;- List of individuals proposed for Card issuance;- Decision on assignment of officials to perform specialized inspection tasks (original copy);- Decision on the official's appointment to rank or leadership position;- Photos of the official proposed for Card issuance (02 photos, 20mm x 30mm) with the full name and the agency written on the back of the photo and on the envelope containing the photos;- Curriculum vitae;- Certificate of training in inspectorate skills or specialized inspection skills that is still valid and issued by a competent authority.
