- Application form for determining or re-determining the disability level and issuance, re-issuance, or replacement of the Disability Certificate.- Copies of documents related to the disability (if any) such as medical records, examination and treatment documents, surgery documents, old Disability Certificate, and other related documents.- Copies of the Medical Assessment Council's conclusion on the ability to serve oneself, the degree of reduction in labor capacity for cases where the conclusion of the Medical Assessment Council was made before the effective date of Decree 28/2012/ND-CP or other related documents (if any).

Notes for cases:

- Needing to process the replacement of the Disability Certificate:- Loss of Disability Certificate

In these cases, the following documents are not required:
