Assessment of primary school students involves activities such as observation, monitoring, exchange, inspection, and commentary on the students' process of learning and training; counseling, guiding, and encouraging students; providing qualitative or quantitative comments on the learning and training outcomes, the formation, and development of certain abilities and qualities of primary school students.

According to the provisions of Circular 30/2014/TT-BGDDT and Circular 22/2016/TT-BGDDT, the assessment of primary school students during the school year by teachers is conducted as follows:
1. Regular Assessment
According to Article 6 of Circular 30/2014/TT-BGDDT as amended in Clause 3, Article 1 of Circular 22/2016/TT-BGDDT, regular assessment involves evaluating the student's learning and training process concerning knowledge, skills, attitudes, and certain manifestations of competencies and qualities. This assessment is conducted according to the content progress of subjects and educational activities. Regular assessment provides feedback information for teachers and students to promptly support and adjust, thereby promoting student progress according to primary education objectives.
When regularly assessing learning, teachers should verbally point out to students what is correct and incorrect and how to correct it; write comments in the student's notebooks or learning products when necessary, and take specific measures to promptly help. In the regular assessment of competencies and qualities, teachers will base their comments on the manifestations of students' awareness, skills, and attitudes in each competency and quality, and take prompt supporting measures.
2. Periodic Assessment of Learning Outcomes
According to Article 10 of Circular 30/2014/TT-BGDDT as amended in Clause 4, Article 1 of Circular 22/2016/TT-BGDDT, periodic assessment evaluates students' educational outcomes after a learning and training period to determine the level of task completion compared to the knowledge and skill standards specified in the primary education curriculum and the formation and development of student competencies and qualities.
Periodic assessment of learning is conducted as follows:
- In the middle of the first semester, at the end of the first semester, in the middle of the second semester, and at the end of the school year, teachers base on regular assessment processes and knowledge and skill standards to evaluate students in each subject and educational activity according to the following levels:
- Well-completed: meeting the learning requirements of the subject or educational activity well;- Completed: meeting the learning requirements of the subject or educational activity;- Not completed: not meeting some learning requirements of the subject or educational activity.
- At the end of the first semester and the school year, periodic tests are conducted for the following subjects: Vietnamese, Mathematics, Science, History and Geography, Foreign Language, Informatics, Ethnic Language.
In grades 4 and 5, there are additional periodic tests in Vietnamese and Mathematics in the middle of the first semester and the second semester.
- Periodic test questions should match the knowledge and skill standards and developmental orientation, including questions and exercises designed according to the following levels:
- Level 1: recognizing and recalling the learned knowledge and skills;- Level 2: understanding the learned knowledge and skills, presenting, explaining the knowledge based on personal understanding;- Level 3: applying the learned knowledge and skills to solve familiar and similar problems in learning and life;- Level 4: flexibly applying the learned knowledge and skills to solve new problems or provide reasonable feedback in learning and life.
- The test is corrected, commented on, and graded by the teacher on a 10-point scale, without giving a 0 or decimal points, and returned to the students.
Periodic test scores are not used to compare one student with another.
If the results of the end-of-term 1 and end-of-year tests are unusual compared to regular assessments, the teacher may suggest the school give the student another test to correctly assess their learning outcomes.
Periodic assessment of competencies and qualities is conducted as follows:
In the middle of the first semester, at the end of the first semester, in the middle of the second semester, and at the end of the school year, the homeroom teacher bases their comments on students' awareness, skills, and attitudes during regular assessments of forming and developing each competency and quality, and summarizes them as follows:
- Good: well-meeting educational requirements, clear and frequent manifestations;
- Met: meeting educational requirements, but not frequently manifested;
- Need improvement: not fully meeting educational requirements, unclear manifestations.
3. Consideration of Class Program Completion and Primary Education Program Completion
According to Article 14 of Circular 30/2014/TT-BGDDT as amended in Clause 7, Article 1 of Circular 22/2016/TT-BGDDT, students are confirmed to have completed the class program if they meet the following conditions:
- Periodic assessment of learning outcomes for each subject and educational activity at the end of the school year: Well-completed or Completed;- Periodic assessment of each competency and quality at the end of the school year: Good or Met;- The final year periodic test scores for each subject must be 5 or above.
For students not confirmed to have completed the class program, teachers plan, guide, assist, and conduct additional assessments to consider class program completion.
For students who have been guided and assisted but still do not meet the conditions to complete the class program, teachers will compile a list and report to the principal for consideration of promotion or retention based on the extent of incomplete subjects, educational activities, and competency and quality development.
Students who complete the grade 5 program are confirmed and noted in the student report book: Completed Primary Education Program.
