On December 30, 2019, the Minister of Health signed and promulgated Circular 37/2019/TT-BYT regulating reporting and statistical policies of the health sector.
Initial record book and health sector statistical report templates are issued with Circular 37/2019/TT-BYT, specifically include:

- Templates and instructions for recording in the initial record book in Appendix I.
- Templates and instructions for recording health statistical reports at the commune level (hereinafter referred to as commune) in Appendix II.
- Templates and instructions for recording health statistical reports at the district level, including towns and provincial cities (hereinafter referred to as district) in Appendix III.
- Templates and instructions for recording health statistical reports at the provincial level, including centrally-run cities (hereinafter referred to as province) in Appendix IV.
- Templates and instructions for recording central health statistical reports in Appendix V.
The reporting unit is responsible for compiling statistical information according to the prescribed report templates in the statistical reporting regulations; the reporting unit is required to review, resubmit reports and provide information related to the statistical report when necessary to check the accuracy of the reported data; ensure the publication, use, and confidentiality of statistical information in accordance with the law.
Additionally, Circular 27/2014/TT-BYT dated August 14, 2014, of the Ministry of Health regulates the Statistical Report Template System applicable to provincial, district, and commune health facilities, Decision 3266/QD-BYT dated September 3, 2008, on issuing central level report templates and Decision 2360/1997/QD-BYT dated November 14, 1997, issuing hospital statistical report templates according to the health sector indicator system, are null and void as of the effective date of Circular 37.
