Depending on the specific subjects, the documentation for confirming the status of a martyr will differ, specifically as stipulated in Circular 202/2013/TT-BQP as follows:

First case: For military personnel, defense public employees, and cryptographic service personnel who died in the line of duty
Documentation: 03 sets (kept at the unit issuing the death notification; the unit directly under the Ministry of Defense; the Department of Labor, War Invalids and Social Affairs where management and implementation of policies occur), each set includes:
- Death Notification (Form LS1), Condolence Letter (Form LS1a), Inventory of personal belongings and money (Form LS1b), Handover record of personal belongings and money (Form LS1c), Grave map (Form LS1d);- Documents serving as the basis for issuing a death notification as stipulated in Article 7 of Circular 202/2013/TT-BQP;- Declaration of family status of the martyr, confirmed by the commune-level People's Committee of the place of residence (Form LS4);- Official Dispatch requesting from the head of the agency or unit as stipulated at Point d, Clause 3, Article 3 of Circular 202.
Second case: For persons missing or lost as stipulated at Point k, Clause 1, Article 17 of the Decree 31/2013/ND-CP and individuals who died before December 31, 1994
Documentation: 03 sets (kept at the unit issuing the death notification; the unit directly under the Ministry of Defense; the Department of Labor, War Invalids and Social Affairs where management and implementation of policies occur), each set includes:
- Application from the family of the subject, confirmed by the commune-level People's Committee of the place of residence (Form LS3);- Verification form for missing or lost cases (Form LS2);- Death Notification (Form LS1);- Documents serving as the basis for issuing a death notification as stipulated in Clause 9 and Clause 11, Article 7 of this Circular;- Declaration of family status of the martyr, confirmed by the commune-level People's Committee of the place of residence (Form LS4);- Official Dispatch requesting from the head of the agency or unit as stipulated at Point d, Clause 3, Article 3 of Circular 202/2013/TT-BQP.
