Circular 11/2014/TT-BVHTTDL stipulates the dossier for requesting the Commendation Medal "For the cause of Culture, Sports, and Tourism" in Vietnam includes:

The unit's proposal form in accordance with Form No. 1 as stipulated in the Appendix attached to this Circular;
Minutes of the unit's Emulation and Commendation Council meeting;
List of individuals proposed for the Commendation Medal;
Summary of the individual's work process and achievements;
Certified copies of commendation decisions for individuals being proposed for the Commendation Medal as stipulated in Clause 2, Article 6 and Clause 2, Article 8 of this Circular.

Note: The dossier is to be prepared in 01 set, using Times New Roman font, submitted directly or via post to the Ministry of Culture, Sports and Tourism's Emulation and Commendation Council (hereinafter referred to as the Ministry's Emulation and Commendation Council) before March 1 or before July 1 annually. For subjects who meet the standards as stipulated, the submission time for the dossier is not necessarily according to this regulation.
