Application for the settlement of canceling the decision on Social Insurance benefits for military personnel in Vietnam (Internet image)
According to Article 28 of Circular 136/2020/TT-BQP, the application for the settlement of canceling the decision on Social Insurance benefits for military personnel; personnel receiving salaries as military personnel; defense officials, workers, and officers; contract workers in agencies, units, and enterprises under the Ministry of Defense is regulated as follows:
* For personnel receiving salaries:
- Application for canceling the decision on Social Insurance benefits (in case the worker requests; Form 14-HBQP);
- Request letter from the Head of the unit under the Ministry;
- Decision on canceling the decision to leave the job to receive Social Insurance benefits (pension, survivor benefits, discharge, resignation) by the authorized Head of the unit;
- Social Insurance file already resolved by the Ministry of Defense Social Insurance; in case of retirement, the file is handed over by the Ministry of Defense Social Insurance to the unit but has not been submitted to the local Social Insurance;
- Decision on canceling the decision on Social Insurance benefits by the Director of the Ministry of Defense Social Insurance (Form 06B-HBQP);
- Receipt of Social Insurance benefits for one-time survivor benefits, discharge, resignation (if any) issued by the unit at the battalion level and above and submitted to the higher financial agency for the unit to transfer to the specialized Social Insurance account of the Ministry of Defense Social Insurance.
* For non-commissioned officers and soldiers:
- Application for canceling the decision on Social Insurance benefits (in case the worker requests; Form 14-HBQP);
- Decision on canceling the decision on Social Insurance benefits and removing the name from the list of recipients, one-time Social Insurance benefits list from the Social Insurance fund by the authorized Head of the unit under the Ministry;
- In case the non-commissioned officers and soldiers are discharged and receive one-time Social Insurance benefits from the Social Insurance fund, which has been settled with the Ministry of Defense Social Insurance by the unit under the Ministry, the file should be accompanied by a receipt of Social Insurance benefits issued by the unit at the battalion level and above and submitted to the higher financial agency for the unit to transfer to the specialized Social Insurance account of the Ministry of Defense Social Insurance;
- Document from the Ministry of Defense Social Insurance regarding the cancellation of the decision on one-time Social Insurance benefits.
