Application for settlement of preservation of Social Insurance payment period for servicemen in Vietnam (Internet image)

According to Article 26 of Decree 136/2020/TT-BQP, the application for settlement of preservation of Social Insurance payment period for servicemen; essential personnel receiving salaries like servicemen; defense officials, employees, and defense public employees; contracted employees in agencies, units, and enterprises under the Ministry of Defense is specified as follows:

* For employees participating in Social Insurance:

• Social Insurance record book;

• Decision on demobilization, discharge, or termination of employment by the competent authority or document terminating the employment contract, employment contract expiration;

For non-commissioned officers and soldiers who have been discharged and do not receive a lump-sum Social Insurance benefit and wish to preserve the payment period, they need to submit a supplementary request form (Form 14-HBQP).

* For cases where individuals who have been demobilized and returned to their localities within 12 months and have received a lump-sum Social Insurance benefit from the Social Insurance, and wish to preserve the payment period:

• Social Insurance record book (if available);

• Request form for preserving the payment period of Social Insurance (Form 14-HBQP);

• Request letter from the head of the unit under the Ministry;

• Social Insurance benefit file that has been settled by the Social Insurance Fund under the Ministry of Defense;

• Decision on revoking the decision on Social Insurance benefits by the Director of the Social Insurance Fund under the Ministry of Defense (Form 06B-HBQP);

• Receipt of lump-sum Social Insurance benefit for demobilization, discharge issued by the battalion-level unit or equivalent, submitted to the higher-level financial agency for transfer to the specialized Social Insurance collection account of the Social Insurance Fund under the Ministry of Defense.

* For individuals serving imprisonment, when being released and transferred to their locality:

• Social Insurance record book;

• Certificate of completion of the prison sentence (copy);

• Request letter from the head of the unit managing the employees before the imprisonment;

• Personal file (original) and related documents regarding the period and wages for Social Insurance payment (if a Social Insurance record book has not been issued).
