Specifically, Article 18 of Decree 94/2009/ND-CP stipulates that forty-five days before the mandatory rehabilitation period ends, the Director of the Center for Treatment - Education - Social Labor is responsible for preparing a file, organizing a Review Council for individuals at high risk of relapse, and preparing an application file proposing post-rehabilitation management measures at the Center. The application file proposing post-rehabilitation management measures at the Center includes:

1. The file of the mandatory rehabilitant at the Center for Treatment - Education - Social Labor (copy);
2. Self-assessment report on the learning and training process during the time at the Center by the rehabilitant, with comments and evaluation from the Director of the Center for Treatment - Education - Social Labor;
3. The minutes of the Review Council for individuals at high risk of relapse at the Center; supporting documents proving the individual falls under the post-rehabilitation management category at the Center;
4. A written proposal from the Director of the Center for Treatment - Education - Social Labor addressed to the Head of the Labor - Invalids and Social Affairs Department where the file was prepared to send the individual to the Center for Treatment - Education - Social Labor.
