Regarding this issue, LawNet would like to answer as follows:
- The individual's application requesting consideration for awarding the title "Doctor of Merit" includes:
+ Achievement report requesting consideration for awarding the title "Doctor of Merit" according to Form No. 09 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ Copy of the decision on awarding emulation titles and relevant forms of reward;
+ Copy of the certificate or decision of the competent management agency for the initiative or patent (if any);
+ Copy of acceptance record of science and technology tasks; Decision to recognize the accepted results of implementing scientific and technological tasks, the results of evaluation and classification at a pass level or higher; Certification by a competent authority on the effective application, and scope of influence of scientific and technological tasks;
+ Confirmation of direct time working as a technical professional in the medical industry. In the event that a medical facility is dissolved or merged, the immediate superior of that unit will confirm the period of medical technical expertise.
- The individual's application for consideration in awarding the title "People's Doctor" includes:
+ Achievement report requesting consideration for awarding the title "People's Doctor" according to Form No. 08 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ Copy of Decision and Certificate of Conferment of the "Doctor of Merit" title;
+ Documents specified in Points b, c, d, dd, e, g, and h of Clause 1 of Article 14 of Decree 25/2024/ND-CP.
- Individuals requesting to be considered for the title "People's Doctor", "Doctor of Merit" submit 01 application online, in person, or via postal service to the agency or unit currently working or the agency or unit that worked before retirement according to the provisions of law.
- Individuals specified in Clause 1 of Article 2 of Decree 25/2024/ND-CPP are responsible before the law for the legality and accuracy of the information declared in the application. When requested by competent authorities, individuals are responsible for providing originals or original documents for comparison.
- In the event that the personal profile is invalid and needs to be supplemented or edited: the agency receiving the profile will guide the individual to complete the profile. Within 07 working days from receipt of the application, the individual returns the completed application to the receiving agency.
- The lower level council sends 01 set of documents to the upper-level council, including:
+ Proposal for consideration for awarding the title "People's Doctor", "Doctor of Merit" according to Form No. 01 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ List of requests for consideration for awarding the title "People's Doctor", "Doctor of Merit" according to Form No. 02 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ Minutes of introduction and results of consultation according to Form No. 03 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ Meeting minutes and voting results to consider awarding the title "People's Doctor", "Doctor of Merit" according to Form No. 04 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ List of individual achievements proposed for consideration for awarding the title "People's Doctor" according to Form No. 05 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ List of individual achievements proposed to be considered for awarding the title "Doctor of Merit" according to Form No. 06 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ Report on the process of considering and awarding the title "People's Doctor", "Doctor of Merit" according to Form No. 07 specified in the Appendix issued with Decree 25/2024/ND-CP;
+ Decision to establish the Council;
+ Physician records specified in Clause 1 and Clause 2, Article 14 of Decree 25/2024/ND-CP.
- The State Council sends 01 set of documents to the Ministry of Home Affairs for appraisal before submitting them to the Prime Minister for consideration and submission to the President for decision, including:
+ Proposal of the Chairman of the State Council, accompanied by a list of proposals for consideration of gifts;
+ Summary of personal achievements certified by the Chairman of the State Council;
+ Minutes and results of the secret voting of the State Council.
(Article 14 of Decree 25/2024/ND-CP)
- The individual requesting consideration for awarding the title shall send documents to the Grassroots Council no later than July 1 of the year of consideration.
- The grassroots council sends the application to the ministerial and provincial councils no later than August 15 of the year in which the donation is considered.
- The ministerial and provincial councils send the application to the State Council no later than October 15 of the year in which the gift is considered.
- The application for consideration of a paper donation (original) is submitted online, directly, or via postal service to the Standing Council specified in Article 15 of Decree 25/2024/ND-CP.
- Standing Councils at all levels receive and check documents. In case the application does not comply with the regulations in Article 14 of Decree 25/2024/ND-CP, within 10 days, the Standing Council receiving the application must notify the individual and the lower-level Council to supplement the application. After receiving the notice, within 10 days, the individual and the lower-level Council must complete the additional documents.
