The latest form of birth document is the form in Appendix 5 issued with Circular 56/2017/TT-BYT.

2. Instructions on filling out the birth document
The instructions for recording the contents of the birth document are as follows:
- Full name of mother or caregiver: Write the mother's name according to ID card, passport or household registration in capital letters with an accent.
- Year of birth: Enter the year of birth of the mother or caregiver according to the calendar year.
- Place of permanent and temporary residence registration: Record the place of permanent and temporary residence registration according to the 4-level geographical location: Village/hamlet, commune/ward/town, district/district, province/city.
In case foreigners have permanent or temporary residence registration in Vietnam, write the same as Vietnamese people. If a foreigner does not register permanent or temporary residence in Vietnam but lives in a border area and gives birth at a Vietnamese medical facility, write the name of the province and country where they are living.
- Social insurance code/Health insurance card number: Enter the social insurance book number or social insurance code. Recording the social insurance code only applies when the social insurance agency officially announces the use of the social insurance code instead of the social insurance book number.
- Identity card number (ID card)/Passport of the mother or caregiver: Clearly state the issued ID card number; in case there is no ID card but have a Passport, write the passport number. If you do not have an ID card or passport, leave it blank. (The regulation on recording an ID card number or passport number only applies until the social insurance agency officially announces the use of social insurance code instead of health insurance card number.).
- Ethnicity: Clearly state the ethnic name of the mother or caregiver, such as: Kinh, Tay, Nung, Hoa, or other ethnicities.
- Father's full name: Record according to information provided by the patient or the patient's family. The medical facility is not responsible for the accuracy of information about the father's full name.
- When the child was born: record the hour, minute, day, month, and year that the child was born according to the calendar year.
- At: Record the place where the child was born, specifically:
+ In case a child is born at a hospital, write the name of the hospital and the administrative location where the child was born (For example: Nam Dinh province general hospital). In case a child is born at another medical facility, write the name of the medical facility and the 3-level administrative location where the child was born (For example: Lien Bao commune health station, Vu Ban district, Nam Dinh province) ;
+ In case a child is born at home, write down the home and location at 3 levels: commune/ward, district/district, province/city (For example: at home in Lien Bao commune, Vu Ban district, Nam Dinh province);
+ In case the child was born somewhere else, besides the medical examination and treatment facility, also write down the place where the child was born and the location of the 3 administrative levels (For example: giving birth on the road, in Lien Bao commune, Vu Ban district, Nam Dinh province);
+ In case a child is abandoned, clearly state the child was abandoned, the place where the child was found, and the location of 3 administrative levels (For example: the child was abandoned in Lien Bao commune, Vu Ban district, Nam Dinh province);
- Number of births: Record all births, including this birth, premature birth, and stillbirth.
- Number of children currently alive: Record the number of children currently alive, including children born alive this time.
- Number of children born this time: Record the number of children born this time. In case of twins or more births, specify the number of children and issue a separate birth document for each child.
- Child's gender: Specify whether the live-born child is male or female. In cases of deformity, it is unclear whether it is male or female; record it as unknown.
- Weight: Record the weight of the newborn baby weighed in the first hour after birth in grams. (For example: a baby is born with 3500 grams.) If not weighed, leave it blank.
- Child's condition: Clearly state the child's condition at the time of making the birth document: healthy, weak, deformed, or other health-related manifestations (if any). If there is a defect, specify the type of defect, including the child's morphological defects if detected.
- Expected name for the child: Write the expected name for the child. This expected name can be changed when birth is registered.
- Note: In case of giving birth requiring surgery, giving birth to a baby under 32 weeks of age or giving birth to a baby under 32 weeks of age and requiring surgery In the notes section, one of the following must be clearly stated: "birth requires surgery" or "birth under 32 weeks of age" or "surgery, birth under 32 weeks of age".
- Birth attendant: sign, clearly state title, and full name (in case of giving birth at a medical facility). In the case of giving birth at home, write specifically the name and title of the birth attendant (if a medical staff member). For example: If the village midwife helps, write the midwife's and midwife's full names. In case the birth attendant is not a medical officer, only write the full name.
- Date, month, year recorded in birth document: Record according to calendar day, month, year.
- Person recording the ballot: sign, clearly state title, and full name.
- The head of the medical examination and treatment facility or the person authorized by the head of the medical examination and treatment facility may sign and affix the seal of that medical examination and treatment facility.
In the event that the medical examination and treatment facility has only one person competent to examine and sign the hospital discharge certificate, that person only needs to sign and stamp the head of the unit.
In the case of giving birth at home, the baby is abandoned, or giving birth at a place other than a medical facility, the health station issues a birth document through the village health staff, and then the head of the medical station records and confirms, signs, stamps, and clearly states the full name.
- Confirmation from the father, mother, or relative: sign, clearly state the full name, and describe the relationship to the child.
