2. Instructions on how to fill out the application form for lump-sum social insurance benefits in 2023 (Form No. 14-HSB)

(1) Write the full name of the beneficiary;

(2) Write down the phone number. In case the employee does not have a phone, he or she can write down the phone number of a relative when needed, along with the full name and relationship to that person;

(3) Write the full address of the person requesting settlement: House number, alley (nook, alley), street, group (village, hamlet, hamlet), commune (ward, town), district (town, city), province, city; In case the beneficiary authorizes another person to complete the application and procedures, write the address of the authorized person;

(4) In case the authorized person is the person making the application, clearly state: "I am .... authorized to make the application" Otherwise, leave blank. For example: I am Nguyen Van A authorized to make the application"; at the same time, submit the Power of Attorney together.

(5) Check the content of the request for resolution and write down specific information.

(6) In case the application is submitted later than the time of receiving pension or the certificate of waiting to receive benefits is lost, an additional explanation must be provided during the late application period regarding illegal exit or was declared missing by the Court or served a prison sentence (if yes, specify the time of illegal exit, declared missing, or time of serving the prison sentence) or clearly state the loss of certificate-pending benefits and commit to taking responsibility for the explanation content.

(7) In case of other requests, clearly state the content of the request and information related to the request for resolution; for example: in case of inconsistency in information about full name, middle name, first name, date, month, and year of birth between the ID card/passport/citizen identity card and the application for social insurance benefits, clearly state what information there is inconsistency in, and attach a copy of the ID card/citizen identification passport.

(8) Address to receive pension, social insurance benefits, new residence moved to: Clearly state the house number, alley (nook, alley), street, group (village, hamlet, hamlet), commune (ward, town), district (town, city), province, and city. If the payment point has been determined at the place of request, please write clearly.

(9) Check the corresponding boxes to select the form of receiving pensions and benefits.

If you receive a lump-sum benefit in cash, tick next to choose whether the recipient is at the social insurance agency or through a social insurance service organization; If received through an ATM account, additionally write down the account number, bank, and bank branch that opened the account.

(10) In case the application is submitted later than the prescribed deadline, clearly state the reason for the late submission.

3. Conditions for lump-sum social insurance allowance in 2023 in Vietnam

Employees defined in Clause 1, Article 2 the Law on Social Insurance 2014 are entitled to a lump-sum social insurance allowance upon their request when falling in one of the following cases:

- They have reached the retirement age specified in Clause 1, 2 or 4, Article 54 the Law on Social Insurance 2014 but have paid social insurance premiums for under full 20 years, or the age specified in Clause 3, Article 54 the Law on Social Insurance 2014 but have paid social insurance premiums for under full 15 years and do not continue paying voluntary social insurance premiums;

- They settle abroad;

- They get a fatal disease, such as cancer, poliomyelitis, dropsy cirrhosis, leprosy, serious tuberculosis, or HIV infection progressing into AIDS, or other diseases as prescribed by the Ministry of Health;

- Employees defined at Points dd and e, Clause 1, Article 2 the Law on Social Insurance 2014 who are demobilized or cease working without being eligible for pension.
