2. Instructions for filling out the confirmation of leave for social insurance in Vietnam

The confirmation of leave for social insurance shall be recorded and issued by a doctor or physician working in a medical facility and issued to an employee participating in social insurance to take leave for outpatient treatment or to take care of a sick child.

The confirmation of social insurance leave must be fully and clearly written, must not be erased, and must be entirely written in Vietnamese (the content on the 2 copies must be the same).

Upper left corner: Enter the name of the medical examination and treatment establishment: Write the medical examination number in the line below the name of the medical examination and treatment establishment (is the ordinal number of examinations and treatments issued by the medical examination and treatment establishment). If a medical examination and treatment facility has more than one medical examination division, write the number of that medical examination division.

2.1. Patient Information section

(1) The first line: Fill in the full name, date of birth, and date of birth of the patient who is granted the confirmation of leave for social insurance benefits (in capital letters). In the event that there is only the year of birth, write that;

(2) Second line:

Social insurance number: Enter the full social insurance number issued by the Social Insurance Agency
(Applicable only when the official social insurance agency has announced the use of the social insurance number instead of the health insurance card number).

Digital health insurance card: Fill in the card code, including the alphanumeric part, based on the information on the patient's health insurance card, including capital letters (applicable only until the official social insurance agency announces that the social insurance number will be used instead of the health insurance card number).

(3) Third line: specify gender.

(4) Fourth line: Specify the unit where the patient works and pay social insurance according to the information provided by the person visiting the clinic: In case the child is sick, write the name of the unit where the father or mother is working and pay social insurance premiums according to the information provided by the person who comes for medical examination.

2.2. Diagnostics and Treatments section

(1) The content of the diagnosis must be a specific description of the health condition and include the disease name or disease code. In the case of a disease requiring long-term treatment, the recording of the disease code and disease name shall comply with the provisions of Vietnam's Circular 46/2016/TT-BYT;

- In the event of a pregnancy termination, specify the reason for the termination as well as the number of weeks pregnant.

- In the case of nursing care, include the phrase "pregnancy care."

(2) Contents of treatment methods: Record treatment indications. In the event of a pregnancy termination:

- Under 22 weeks old, based on actual condition to write treatment method in one of the following cases:
Except in the case of minimizing pregnancy during in vitro fertilization, miscarriage, abortion, and caesarean section;

- From 22 weeks of age or older, clearly indicate whether it is a normal delivery, a surgical delivery, or a caesarean section.

Determining the week of pregnancy is based on the date of the last menstrual period or the ultrasound results in the first 3 months of pregnancy.

(3) Number of days off: the decision on the number of days off must be based on the patient's health condition, but it must not exceed 30 days for a single grant of a confirmation of leave to enjoy social insurance.

Particularly in the case of a patient being treated for tuberculosis under the national anti-TB program, the maximum leave time shall not exceed 180 days for a confirmation of leave from work entitled to social insurance.

If an employee suffers a miscarriage, abortion, curettage, abortion, or stillbirth with a gestational age of 13 weeks or more, the maximum leave time prescribed by the Social Insurance Law but not exceeding 50 days for a confirmation of leave to enjoy social insurance.

The recording of the start date of leave must coincide with the date of the patient's visit.

2.3. Parent information section

Fill in the full family name, middle name, and first name of the patient's father and mother (if any) in case the patient is a child under 7 years old.

2.4. Confirmation from the unit head

The head of the medical examination and treatment establishment or the person authorized by the head of the medical examination and treatment establishment may sign and seal the medical examination and treatment establishment's seal. If the head of the medical examination and treatment establishment or a person authorized by the head of the medical examination and treatment establishment can sign and seal concurrently as the medical examiner, that person must only sign and stamp in this section and not in the medical section, and the medical examination and treatment doctor must still write the date, month, and year of the issue.

The issuance date, month, and year must coincide with the date the employee arrives for a medical examination; if the medical examination lasts two days or more, the issuance date, month, and year must coincide with the last day of the employee's medical examination, and the employee should be assigned outpatient leave.
