Instruction on filling out the registration form to participate in the people's police service according to the instructions in the Appendix issued with Decree 70/2019/ND-CP as follows:
(1) Color photo (4x6 cm) taken within 06 months from the date of declaration, pasted and stamped.
(2) Full name of birth: Write the correct capital letter with the first and last name in the Birth Certificate.
(3) Date of Birth: Fill in the full day, month, and year of birth exactly as in the Birth Certificate.
(4) Place of birth: Enter the name of the commune (or ward, township), district (or district, town or city of the province),
province (or city directly under the central government) where you were born (recorded exactly as in the Birth Certificate). If there is a change in the name of the administrative unit, write (old name), currently (new name).
(5) Hometown: Record according to Birth Certificate or People's Identity Card or Citizen's Identity (identification of hometown according to the provisions of the law and guidance of the Ministry of Justice). Specify the name of the commune (or ward, township), district (or district, provincial city), or province (or city directly under the central government).

3. Criteria for selection to participate in the people's police service in Vietnam
According to Article 5 of Decree 70/2019/ND-CP, citizens are selected to perform the obligation to join the People's Police when they fully meet the following criteria:
- Having a clear background.
- Strictly abide by the lines and guidelines of the Party, policies, and laws of the State; no criminal record, no criminal prosecution, probation; not during the period of being applied the measure of education in communes, wards, or townships or sent to compulsory medical treatment establishments or compulsory educational establishments;
Having good qualities, ethics, and conduct, being trusted by the masses of people in the place of residence, study, or work.
- Ensuring the political standards of non-commissioned officers and soldiers serving according to the duty regime in the People's Public Security.
- Having a high school diploma or higher. Communes in mountainous areas, islands, ethnic minority areas, and areas with extremely difficult socio-economic conditions may recruit citizens with lower secondary school diplomas.
- Having a well-balanced physique, not deformed or deformed, and meeting the health standards to perform the obligation to join the People's Police.
