Specifically, compared to the current Health Insurance Card (HI Card) issued under Decision 1313/QD-BHXH, the new HI card template has the following notable features:

Size: with a length of 98 mm, width of 66 mm, following the outer frame border of the card (enough to print 08 HI card samples on 01 page, A4 paper size).

Size: length of 85.60 mm, width of 53.98 mm, following the outer frame border of the card

Material: using white paper with a weight of 150g/m2, ensuring durability, glossiness, and the ability to print the participant's photo for HI.

Material: using white paper with a weight of 180g/m2, ensuring durability and glossiness.

Appearance: white background, with a faintly printed Vietnam Social Insurance symbol in light blue with spreading circles, gradually decreasing in color intensity, with a distance of 12 mm from the center of the Vietnam Social Insurance symbol to the outermost circle; outside there is a double border frame in light blue.

** Front side:

* Adjacent to the left margin of the HI card:

- At the top, there is a light blue Vietnam Social Insurance symbol with a diameter of 10 mm, using anti-counterfeit reflective material.

- Below is the position for pasting or printing the participant's photo for HI.

- At the bottom is the position for printing the barcode.

* Adjacent to the right margin of the HI card:

- At the top, there is a line of text "BẢO HIỂM XÃ HỘI VIỆT NAM" (Vietnam Social Insurance) with a light blue underline, bold Times New Roman font, size 10.

- Below is the line of text "THẺ BẢO HIỂM Y TẾ" (Health Insurance Card) in red, bold Times New Roman font, size 12.

- Along the right edge of the HI card, the "serial number" of the card sample is printed in red, using anti-counterfeit reflective material, Times New Roman font, and a "co" index from 10 to 12, consisting of 10 characters, of which: the first two characters are numbers (from 01 to 99) corresponding to the province code specified in Decision 124/2004/QD-TTg dated July 8, 2004, issued by the Prime Minister, except for the first two characters of the card sample issued by the Ministry of National Defense's Social Insurance is 97, and the one issued by the People's Public Security's Social Insurance is 98. The next 08 characters are numbers (naturally from 00000001 to 99999999).

- The seal of the Social Insurance of provinces and centrally-run cities (hereinafter referred to as provincial Social Insurance) is pre-printed on the HI card sample, with a diameter of 21 mm in red, using anti-counterfeit reflective material. The position from the inner edge of the right border frame of the card to the outer edge of the seal is 28 mm, and from the inner edge of the lower border frame of the card to the outer edge of the seal is 2 mm.

** Back side:

- At the top, there is a line of black text "NHỮNG ĐIỀU CẦN CHÚ Ý" (THINGS TO NOTE), Times New Roman font, size 12.

- Below is a line of black text, Times New Roman font, italic, size 10, with the following content:

"1. The HI card is the basis for determining the rights and benefits of the cardholder for health insurance.

2. Each participant in HI is issued one card and one unchanged identification number throughout their participation in HI.

3. When seeking medical treatment, the HI card with a photo must be presented; if there is no photo, it must be presented along with personal identification documents with photos. Only children under 6 years old need to present the HI card.

4. Handle with care, do not lend the card to others, and remember the identification number on the HI card.

5. In case of loss or damage, when reissuing or replacing the card, the applicable fee must be paid according to regulations."

 Appearance: white background, with a faded logo of the Social Insurance (SI) symbol in light blue with spreading circles, decreasing in color intensity. The distance from the center of the Vietnam Social Insurance symbol to the outer circle is 14 mm. There is a double blue border frame on the outside.

** Front side:

* Adjacent to the left margin:

At the top, the logo of the SI Sector, with a diameter of 07 mm, is printed using reflective material (to prevent counterfeiting).

* Adjacent to the right margin:

- At the top, there is the text "BẢO HIỂM XÃ HỘI VIỆT NAM" (Vietnam Social Insurance) and a blue baseline, using the Tahoma font, uppercase, bold, font size 07.

- Below that is the text "THẺ BẢO HIỂM Y TẾ" (Health Insurance Card) in red, using the Tahoma font, uppercase, bold, font size 09.

- The Vietnam Social Insurance emblem is pre-printed on the HI card, with a diameter of 15 mm, in red, using reflective material. The distance from the inner edge of the right border of the card to the outer edge of the emblem is 16 mm, and from the inner edge of the bottom border of the card to the outer edge of the emblem is 01 mm.

** Back side:

- At the top, there is the text "NHỮNG ĐIỀU CẦN CHÚ Ý" (Important Notes) in black, using the Tahoma font, uppercase, bold, font size 09.

- Below that, there are black texts, using the Tahoma font, lowercase, italic, font size 07, with the following content:

"1. When seeking medical treatment, the HI card with a photo must be presented. If there is no photo, it must be presented together with a valid personal identification document with a photo. Only the HI card needs to be presented for children under 6 years old.

2. Each participant in the HI program is issued a card with a unique SI number. Use the SI number and register transactions at the address: https://dichvucong.baohiemxahoi.gov.vn to continue participating in HI, renew or replace the HI card, and check healthcare expenses covered by HI.

3. To obtain information about the HI card, visit the website: https://baohiemxahoi.gov.vn or send a message in the format: BH THE {SI number} to 8079.

4. For any difficulties, please contact the provincial or district SI office where the card was issued, or call the hotline 19009068 for support and assistance.

5. The HI card must be carefully preserved and not lent to others. In case of loss or damage, promptly report to the SI office."

- Along the right border of the HI card, the "serial number" of the HI card template is printed in red, using reflective material, Tahoma font, with a "co" index ranging from 6 to 8, consisting of 10 characters (naturally numbered from 0000000001 to 9999999999).

The information printed on the HI card template when issued to users includes:

1. Management criteria for HI participants.

1.1. HI card number: indicates the participant's HI card number issued according to the regulations of the Vietnam Social Insurance (SI).

1.2. Full name: prints the participant's full name in capital letters, as stated in the HI registration file.

1.3. Date of birth: prints the participant's date, month, and year of birth according to the HI registration file.

1.4. Gender: indicates male or female based on the HI registration file.

1.5. Location code of the place of residence: indicates the code of the area where ethnic minorities and poor households are living in economically and socially difficult conditions, areas with particularly difficult economic and social conditions, and the code of the area where the HI participant is living in island communes or districts.

1.6. Address: prints the address of the permanent or temporary residence of the person named on the HI card. For children under 6 years old, the address of the mother, father, or guardian is printed. In the case of abandoned children, the address of the foster parent or temporary care organization is printed.

1.7. Initial registration healthcare facility: prints the name and code of the healthcare facility where the HI participant initially registered for medical examination and treatment.

1.8. Validity period: prints the validity period from the date stated in the current regulations of the Vietnam Social Insurance.

1.9. Time of continuous 5-year participation: indicates the date ..../..../..... when the HI participant has completed 5 years of continuous participation, specifically:

a) In case the HI participant is entitled to have 100% of healthcare expenses paid by the HI fund according to the regulations of the amended Law on HI or the continuous participation period is less than 5 years, this criterion is left blank (not printed).

b) In case the HI participant has participated continuously for more than 5 years, the time of continuous 5-year participation is printed as follows:

- For those who have completed 5 years of continuous HI participation until January 1, 2015, the date January 1, 2015 is printed.

- From January 1, 2015 onwards, for those who start to have continuous 5-year participation, the date of the first day of the 6th year is printed.

1.10. Father (mother): only printed on the HI card issued for children under 6 years old, following the priority order as follows: the mother's full name is printed; if there is no mother, the father's full name is printed; if there is no father or mother, the guardian's full name is printed (individual or representative of a legally prescribed organization or appointed to take care and protect the lawful rights and interests of the child).

2. Position for attaching photo: includes an empty black frame with a size of 20mm x 30mm. For children under 6 years old, no photo is required, so the information about children under 6 years old is shifted to the left side (where the photo is printed or attached) to maintain balance with the HI card template.

3. Position for printing barcode: the barcode printed on the HI card is a two-dimensional barcode containing the information on the card and a check character string. It is printed in the space adjacent to the left margin of the HI card and below the position for printing or attaching the photo of the HI participant.

4. Printing ink quality: uses the original ink from the manufacturer (no reused ink) suitable for the types of printers used by SI at all levels to print HI cards.

The information printed on the HI card template and the management requirements when issued to users include:

1. Management criteria for HI participants

1.1. ID number: prints a 10-character SI ID number of the HI participant.

1.2. Full name: prints the full name of the HI participant in capital letters.

1.3. Date of birth: prints the date, month, and year of birth of the HI participant.

1.4. Gender: prints male or female based on the HI registration file.

1.5. HI entitlement level code: prints a (01 character: in order from 1 to 5) code indicating the entitlement level of the HI participant.

1.6. Location code of the place of residence: prints a (02 characters: K1/K2/K3) code indicating the area where ethnic minorities and poor households participate in HI, living in economically and socially difficult conditions, areas with particularly difficult economic and social conditions, and the code of the area where the HI participant is living in island communes or districts.

1.7. Initial registration healthcare facility: prints the name of the healthcare facility where the HI participant initially registered for medical examination and treatment.

1.8. Utilization value: prints the usage value of the card from the date .../.../...

1.9. Time of continuous 5-year participation: indicates the date ....../......../....... when the HI participant has completed 5 years of continuous participation.

1.10. Father (mother): prints the full name of the participant's father or mother, depending on the priority order, for children under 6 years old.

2. Position for attaching photo: includes an empty black frame with a size of 20mm x 30mm. For children under 6 years old, no photo is required.

3. Position for printing barcode: the barcode is printed in the space adjacent to the left margin of the HI card and below the position for attaching the photo.

4. Printing ink quality: uses original ink from the manufacturer suitable for the types of printers used by SI to print HI cards.

The case of individuals using the HI card Decision 1313/QD-BHXH is handled as follows:

- HI card templates that have been printed according to the regulations of Decision 1313/QD-BHXH and have not been fully used until the effective date of Decision 1666/QD-BHXH (from April 1, 2021) will continue to be issued to participants in the HI program.

- During the waiting period for changing the HI card according to the regulations of Decision 1666/QD-BHXH, the HI cards issued to participants with remaining validity can still be used for healthcare examination and treatment.

- Information about the HI card number (as regulated by the Vietnam Social Insurance in Decision 1351/QD-BHXH dated November 16, 2015, and current guidelines on HI card numbers), along with the address (residential or workplace) and the period of exemption from payment within the scope of HI benefits when receiving healthcare examination and treatment at the designated facility, will be updated on the Data Reception Portal of the HI Assessment Information System and the Vietnam Social Insurance Portal.
